Wrinkle relaxer vs filler which line needs which treatment
Wrinkle-relaxing injections and dermal fillers are often lumped together, but one quietens a muscle and the other replaces lost support. Knowing which kind of line you have is most of the decision.
Quick answer
- Wrinkle-relaxing injections soften lines caused by muscle movement, such as frown lines, forehead lines and crow's feet. Dermal fillers replace lost volume and support, for example in hollow cheeks or temples, a weak chin or deep folds.
- Wrinkle-relaxing injections start working within a few days, reach their full effect at about two weeks and usually wear off over about three to four months. Hyaluronic acid filler works straight away and commonly lasts six months to two years, depending on the area.
- Many faces need both, and combined treatment is now common practice, with each treatment matched to the cause of the line rather than to the line itself.
- Both are medical treatments performed by Dr Sin Yong at IN Eternity Clinic and priced at consultation. For fine, crepey surface lines, SkinRev Lab offers non-invasive options such as SKINTIGHT (S$588 per zone) and the Rejuran PDRN Facial (from S$128), before GST.
The difference
One relaxes a muscle, the other replaces support
Wrinkle-relaxing injections use a prescription neuromodulator. Small doses placed into specific muscles reduce the nerve signal that makes them contract, so the skin above is folded less and the line softens. Neighbouring muscles keep working, which is how natural expression is kept.
Dermal fillers are mostly hyaluronic acid (HA) gels, regulated as medical device implants. Placed in the right layer, the gel occupies space and binds a little water, restoring support where the face has lost volume or never had much projection. Filler does not stop a muscle moving.
So a relaxer cannot fill a hollow, and a filler is a poor tool for a line made purely by movement. Most confusion comes from expecting one to do the other's job.
Dynamic vs static
Dynamic lines, static lines and folds
Try this in a mirror. Frown, raise your brows and squint, then relax. Lines that appear only with movement are dynamic lines, made by muscle; these are what wrinkle-relaxing injections are designed for.
Lines still visible when your face is completely at rest are static, or etched. Relaxing the muscle can stop them deepening and soften them partly, but etched lines often also need treatment for skin quality, such as boosters or lasers.
Folds and hollows, such as deep smile lines from nose to mouth, sunken temples or flat cheeks, come mainly from lost volume and support. These are where filler, or a collagen biostimulator, is usually considered, often by supporting the area above the fold rather than filling the crease itself.
By area
Which areas suit which
| Area or concern | Usually suits | Notes |
|---|---|---|
| Frown lines between the brows | Wrinkle-relaxing injections | A deep etched groove may need more than one approach |
| Horizontal forehead lines | Wrinkle-relaxing injections | Extra care if brows sit low or upper lids feel heavy |
| Crow's feet | Wrinkle-relaxing injections | Crepey skin around the eyes may also need skin-quality care |
| Square jaw from a strong chewing muscle | Jaw slimming injections (a neuromodulator) | Only helps if muscle, not bone or fat, is the cause |
| Hollow cheeks or temples | Dermal filler or collagen biostimulator | Restores lost support |
| Short or receding chin | Dermal filler | Firm gel on bone; also balances the profile |
| Deep smile lines (nasolabial folds) | Often cheek support, sometimes filler in the fold | Relaxers are not used for this fold |
| Under-eye hollows | Tear trough filler, in selected people | Not for puffy eye bags |
| Fine, crepey lines on the cheeks | Skin boosters, lasers or non-invasive resurfacing | Neither injection alone is the answer |
Timing
Onset, results and how long each lasts
How long either lasts depends on dose or amount, the strength of your muscles, the product and the area. Timing for repeat treatment is best planned at review rather than on a fixed calendar.
| Wrinkle-relaxing injections | Dermal filler (HA) | |
|---|---|---|
| Starts working | Within a few days | Straight away |
| Full result | About two weeks | Once swelling settles; reviewed a few weeks later |
| Typical duration | Usually about three to four months; varies | About six months to two years, depending on area |
| Session | About 10 to 20 minutes | About 30 to 60 minutes |
| Usual downtime | Small bumps for 15 to 30 minutes; occasional pinpoint bruise | Swelling, tenderness or bruising for a few days |
| If you dislike it | Wears off as muscle activity returns | HA can be dissolved with hyaluronidase |
Combining
Using both together
A global consensus of plastic surgeons and dermatologists described combined treatment with fillers and neuromodulators as a standard of care, with the aim of balancing the face as a whole rather than chasing single lines. Where lost volume makes a muscle work harder, lower doses that soften rather than freeze are favoured.
A typical combination is relaxing the frown muscle while supporting an etched line or a flat temple, or treating crow's feet while restoring cheek support. Whether both are done at one visit or staged is the doctor's call, based on your face.
Faces differ, too. A pan-Asian consensus noted that facial shape, anatomy and aesthetic goals in Asian patients can call for different indications and dosing, such as treatment of the jaw muscle. A consultation that looks at your face at rest and in motion matters more than any general rule.
Safety
Side effects and who should wait
Wrinkle-relaxing injections: small bumps and pinpoint bruises are common, and headache can occur. The unwanted effect people worry about most is a heavy brow or drooping eyelid, which is temporary and made less likely by careful assessment and placement. In a meta-analysis of placebo-controlled trials for frown lines, adverse events were no more frequent than with placebo.
Dermal fillers: swelling, tenderness and bruising are common; lumps, a bluish tint under thin skin, delayed swelling and migration are less common. The most serious risk is vascular occlusion, where filler blocks a blood vessel. It is rare, but skin turning pale, blotchy or dusky, increasing pain or any change in vision after filler is an emergency.
- Pregnant or breastfeeding: both are usually postponed.
- Neuromuscular conditions such as myasthenia gravis: wrinkle-relaxing injections are not suitable.
- Active infection, cold sore or acne at the site: wait until it has cleared.
- Blood-thinning medicine or supplements: tell the doctor, as bruising is more likely.
- Certain antibiotics and other medicines can interact with neuromodulators: bring a list.
Non-invasive
When neither injection is the answer
Fine lines from dehydration, thin skin or rough texture are a skin-quality problem rather than a muscle or volume problem. Non-invasive care can soften them, and it is a sensible place to start in your twenties and early thirties.
At SkinRev Lab, CityLink Mall: SKINTIGHT resurfaces and tightens the surface layer at S$588 per zone; the needle-free Rejuran PDRN Facial supports overall skin quality from S$128 to S$348; and the Needleless SkinBooster hydrates at S$288 per region, with the first region from S$98. All prices are before GST. None of these relaxes a muscle or adds volume, but they work well alongside medical treatment. Daily sunscreen matters as much as any of them.
Where
Getting assessed
Wrinkle-relaxing injections and dermal fillers are prescription and medical treatments, performed by Dr Sin Yong at IN Eternity Clinic, 9 Scotts Road, #12-01 Pacific Plaza, beside Orchard MRT. Your face is assessed at rest and in motion, and the plan and fees are explained before anything is done; both are priced at consultation. WhatsApp +65 8815 3008.
SkinRev Lab does not inject. If you start at CityLink Mall, our Aesthetic Specialist can tell you whether a non-invasive option fits or whether the doctor is the better first step, and pass on your enquiry. The two locations are three MRT stops apart.

Common questions
Your questions,
answered
Is a wrinkle relaxer or filler better for forehead lines?
Usually a wrinkle relaxer, because forehead lines are made by the muscle that lifts the brows. If the lines stay at rest, skin-quality treatment may be added; filler is rarely the first choice here.
Which is better for smile lines?
Deep smile lines from nose to mouth come mainly from lost support and descent of the cheek, so filler or other support is considered rather than a wrinkle relaxer. Often the cheek is supported first, with less product near the fold.
Can I have wrinkle relaxers and filler on the same day?
Sometimes. Combined treatment is common, but whether both are done at one visit or staged depends on the areas and your face. The doctor will advise at consultation.
Which lasts longer, wrinkle relaxers or filler?
Filler usually lasts longer: HA filler commonly lasts six months to two years depending on the area, while wrinkle-relaxing injections usually wear off over about three to four months.
Will wrinkle-relaxing injections make my face look frozen?
Not if the dose and placement suit your face. The aim is to soften the movement that creases the skin while keeping expression, and the result can be adjusted at the two-week review.
Can filler fix crow's feet?
Crow's feet are mainly made by movement, so wrinkle-relaxing injections are the usual first step. Crepey skin around the eyes may also benefit from skin-quality treatment; filler near the eyes is used cautiously and only for specific problems.
What age should I start wrinkle-relaxing injections?
There is no set age. Some people in their late twenties or thirties choose light treatment to slow the deepening of expression lines, but it is not essential, and sunscreen and skincare matter just as much.
How much do wrinkle relaxers and fillers cost in Singapore?
At IN Eternity Clinic both are priced at consultation, because the plan depends on the areas, your muscles and the amount of product. At SkinRev Lab, SKINTIGHT is S$588 per zone and the Rejuran PDRN Facial is S$128 to S$348, before GST.
