A double chin is the soft fullness under the jaw that blurs the angle between chin and neck. It shows most when you look down, on video calls and in side-profile photos. People of every weight get it. Fat under the chin sits in two layers. The upper layer lies just beneath the skin, above a thin sheet of neck muscle called the platysma. The deeper layer lies under that muscle. A 2025 systematic review and meta-analysis pooled eight before-and-after studies of cryolipolysis for submental fat in 206 people. On average, fat thickness under the chin fell by about 2.8 mm and volume by about 20 cm³, with mostly mild and short-lived side effects. A wider systematic review of cryolipolysis adverse events listed redness, numbness, bruising and swelling as the common effects, and named rarer but serious ones such as persistent pain, nerve problems and paradoxical adipose hyperplasia.
Paradoxical adipose hyperplasia means the treated fat grows instead of shrinking. A 2025 meta-analysis of 28 studies estimated it at about 1 in 455 people, higher than manufacturer reports, and called for clear risk disclosure. For tightening, a systematic review of microfocused ultrasound included submental studies that measured a smaller under-chin area on side-view photographs, mainly in mild to moderate laxity. Individual results vary.

Can I get rid of a double chin without losing weight?
Often, yes, if the fullness is local fat or loose skin rather than general weight. Genetics decides much of the fat stored under the chin, so slim people can still have it. If you are carrying extra weight overall, losing some usually changes the area too, and the plan should take that into account.
Does fat freezing under the chin hurt?
Most people feel strong cold, pulling and aching for the first few minutes, which then fades as the area numbs. Afterwards it is common to have redness, swelling, tenderness or numbness that settles over days to weeks. Rarely, the treated fat enlarges instead of shrinking, a condition called paradoxical adipose hyperplasia. Dr Sin Yong will explain these risks before treatment.
How soon will I see a difference?
Tightening and fat reduction both work slowly. Fat cells injured by cooling are cleared over about two to three months, and collagen remodelling after heat treatment follows a similar timeline. You should not expect a visible change on the day. Photos taken before treatment and again after a few months are the fairest way to judge your own response.
