Subcutaneous vs visceral fat: what fat removal can reach

There are two main kinds of body fat, and they respond to different things. Subcutaneous fat sits just under the skin and above the muscle; it is the soft layer you can pinch on the tummy, flanks, thighs, arms, bra line and under the chin. Visceral fat sits deeper, behind the abdominal wall and around the organs, and makes a tummy feel firm and round with little to pinch.

Every fat removal treatment, from microwave and ultrasound to fat freezing, injections and liposuction, works on subcutaneous fat. No device can reach visceral fat, and any treatment claiming to do so is overstating. Visceral fat carries more of the metabolic risk, and it responds to lifestyle change: a 2023 meta-analysis of 40 randomised trials with 2,190 adults with overweight or obesity found that both exercise and calorie restriction reduced visceral fat measured on CT or MRI, with more exercise bringing a larger reduction.

A tape measure helps. HealthHub, Singapore's health portal, advises keeping your waist below 80 cm for Asian women and 90 cm for Asian men. If your waist is above that, or your tummy is firm and hard to pinch, start with weight management rather than fat removal.

Face

Face fat removal: double chin, cheeks and jawline

Face fat removal usually means one of three areas, and the cause decides the option. Under the chin, fat sits in two layers: the pinchable layer above the thin neck muscle, which non-invasive devices, fat freezing and standard liposuction reach, and a deeper layer beneath it, which they do not. Loose skin, a set-back chin or the neck glands can add fullness that fat removal will not change.

In the cheeks, the buccal fat pad lies deep beneath the cheek muscles, close to the inside of the mouth. No facial, massage or device reaches it, and buccal fat removal is permanent surgery with little long-term follow-up; facial fat also thins naturally with age, so removing it young may leave the cheeks hollow later. A wide lower face is often a large chewing muscle rather than fat, which jaw slimming injections, a wrinkle-relaxing injection performed by Dr Sin Yong at IN Eternity Clinic and priced at consultation, can temporarily thin over a few weeks.

Stomach

Stomach fat removal: lower tummy and love handles

Stomach fat removal works on the soft, pinchable layer of the lower tummy and the flanks, often the last places to slim. Crunches strengthen the muscles but do not burn the fat on top of them. Start with a pinch test: a thick, soft roll is subcutaneous fat; firm fullness that you cannot pinch, mostly above the belly button, is visceral fat and a weight-management question.

A fold of skin that hangs over the waistband, a ridge down the middle of the tummy after pregnancy, or a bulge at the belly button is not a fat-removal case. These need a doctor's or plastic surgeon's assessment, and sometimes a tummy tuck.

By method

Fat removal by method: heat, cold, ultrasound, injections and surgery

Reviews of non-invasive devices describe mild to moderate change, for example around 2 cm or more of circumference across the abdomen, hips and thighs, without serious adverse effects. Results vary between people and between devices. Every method changes shape in a treated area rather than body weight, and fat cells that are removed do not come back, although the remaining ones can grow with weight gain.

Injections

Fat-dissolving injections: what the evidence covers

Fat-dissolving injections use deoxycholic acid, a prescription medicine that destroys fat cells where it is injected; the body then clears them. The strongest evidence is for the double chin. In the REFINE-1 trial of 506 adults, 70% of those treated improved by at least one grade, against 18.6% on placebo, usually over several sessions: 55% had improved after two treatments and 75% after four. Side effects were mostly mild and at the injection site, and temporary weakness of the nerve to the lower lip occurred in 4.3% of treated people, mostly resolving without lasting effects.

Injections for larger body areas have much less evidence, and other mixtures are sold under the same 'lipolysis' label. These are medical procedures for a doctor to assess. SkinRev Lab itself does not prescribe or inject, and fat-dissolving injections are not among the treatments listed for IN Eternity Clinic; a doctor can advise whether they suit you.

Surgery

Liposuction: the surgical option

Liposuction removes fat through small cuts using a thin tube called a cannula, and suits larger amounts of soft fat with skin that still springs back. It removes more in one procedure than any device, but it is surgery: the NHS notes that it is not a treatment for obesity and will not remove cellulite, that a compression garment is worn constantly for several weeks, and that bruising and swelling can take up to six months to settle completely.

Done on its own, it has a low rate of serious problems: in a US database of 31,010 liposuction procedures, major complications within 30 days occurred in 0.7% when liposuction was performed alone, with risk rising when it was combined with other operations and with higher age and BMI. Ask who will operate, where, what the quote includes and how complications would be handled.

When to see a doctor before any fat removal

• The fullness is firm and you cannot pinch it, or your waist is above 80 cm (women) or 90 cm (men) with raised blood pressure, blood sugar or cholesterol.
• A lump feels different from the fat around it, is growing, painful or tender; it may be a lipoma or something else that needs a diagnosis.
• Both legs are full from hip to ankle with slim feet, tenderness or easy bruising, which can be lipoedema.
• A lump or swelling in the armpit changes with your menstrual cycle, which can be breast tissue.
• Your face or tummy has rounded over weeks or months, especially while taking steroid medicines; never stop them on your own.
• There is a bulge at the belly button or along a scar, or a ridge down the middle of the tummy after pregnancy.
• You are pregnant or breastfeeding, or have a pacemaker, another implanted device or metal implants in the area.
