How long do fillers last? by area, and why the filler can outlast the look

Most people are quoted a number of months. The honest answer depends on the area, the gel and your own face, and the visible effect often fades before the filler itself has gone.

Quick answer

  • Hyaluronic acid (HA) filler usually stays visible for about six months to two years, depending on where it is placed and which gel is used. Lips tend to be at the shorter end; cheeks and chin at the longer end.
  • Visible effect and actual presence are different things. MRI studies have found HA filler in the mid-face years after injection, which is why repeated top-ups can build up into an overfilled look.
  • Collagen biostimulators such as PLLA work differently: results build gradually over months and cannot be dissolved, whereas HA filler can be dissolved with an enzyme called hyaluronidase.
  • In Singapore, dermal fillers, filler correction and pillow-face correction are medical treatments; at IN Eternity Clinic they are performed by Dr Sin Yong and priced at consultation.

By area

Typical filler duration by area

These are ranges reported in studies and commonly used in practice, not promises. The product, the amount, the depth and your own tissues all change the answer, and your doctor should tell you what to expect for the specific gel used.

AreaTypical visible durationWhy
LipsAbout 6 to 12 monthsConstant movement and softer gels; in a meta-analysis, 74% still had fuller lips at 6 months and 46% at 12 months
Cheeks and mid-faceAbout 12 to 24 monthsFirmer volumising gels placed deep, often on bone; in one trial nearly half kept their correction at 24 months
Tear trough (under-eye)Often 12 months or longerLittle movement; soft gels can persist, and the area is prone to build-up and puffiness
Chin and jawlineOften 12 months or longerFirm gel on bone in an area that moves relatively little
NoseAbout 8 to 14 months in published reviewsSmall volumes of firm gel; results depend heavily on technique

Visible vs present

Why filler can outlast its visible effect

HA filler is broken down gradually, and the visible fullness fades as the gel loses some of its water and the face keeps ageing underneath. That is not the same as the filler being gone.

In an MRI review of 33 people with mid-face HA filler, filler was still detectable in every one of them at least two years after injection, and in some people for many years. Most had not been treated for several years.

The practical lesson: if you top up every time the look fades, new filler can layer over old filler. Over years this can produce a heavy, wide or puffy look. An assessment of what is already there should come before more volume.

What changes it

What can shorten or lengthen filler

Early swelling also confuses people. Lips and under-eyes in particular can look fuller for the first days after treatment, so a result can seem to 'disappear' quickly when it is really swelling settling. The true result is judged at review, a few weeks later.

  • Movement: lips and the area around the mouth move all day; the chin, temples and under-eyes move less.
  • Gel type: firmer, more cross-linked volumising gels generally last longer than soft gels made for fine lines or lips.
  • Depth and amount: filler placed deep on bone tends to hold its shape longer; very small amounts fade from view sooner.
  • Your own tissues: people break filler down at different rates, and claims that exercise or a particular routine 'burns off' filler are not well supported.
  • Your face changing: ongoing bone, fat and skin changes can make the same filler sit differently over time.
  • Top-ups: adding filler before the earlier filler has gone makes results seem longer-lasting, but it can build up.

HA vs biostimulators

Biostimulators: a different kind of longevity

Collagen biostimulators, such as poly-L-lactic acid (PLLA) and polycaprolactone (PCL), do not simply fill a space. They prompt your own tissue to build collagen over weeks to months, so the change is gradual and usually needs more than one session. In a randomised study of PLLA for nasolabial folds, improvement was reported up to 25 months after the last treatment.

The trade-off is control. HA filler can be dissolved if it is not right; PLLA and PCL cannot be dissolved with hyaluronidase. Biostimulators are also generally not used in the lips.

Dissolving

Dissolving filler with hyaluronidase

HA filler can be broken down with hyaluronidase, an enzyme injected into the unwanted filler. It usually works over roughly one to three days, and the result is reviewed once the tissues have settled. Larger amounts are often treated in stages.

Doctors commonly advise waiting at least two weeks after dissolving before any new filler, and often longer, so proportions can be judged properly. Hyaluronidase is a protein, so allergy is possible; people who react strongly to bee or wasp stings need careful assessment and sometimes a skin test. It does not dissolve non-HA fillers.

Migration

When filler moves: migration, lumps and puffiness

Migration means filler has moved away from where it was placed. Common signs are a ledge or fullness above the lip line, puffiness under the eyes that is worse in the morning, lumps you can feel, a bluish tint under thin skin (the Tyndall effect), or a face that looks wider or heavier than intended.

Causes include filler placed too shallow or in the wrong layer, large volumes, repeated top-ups and the pull of facial muscles. Swelling that appears weeks or months later can also be a delayed inflammatory reaction, sometimes after an illness, vaccine or dental work. These should be assessed rather than left; ultrasound can show where filler sits before anything is dissolved.

  • Seek urgent help if skin near recent filler turns pale, white, blotchy or dusky.
  • Pain that is increasing rather than settling needs urgent attention.
  • Any change in vision after filler is an emergency.
  • Contact the clinic that treated you at once, or go to an emergency department.

Planning

Planning top-ups sensibly

Skin quality changes alongside volume, and non-invasive care can help between medical visits, though none of it adds volume. At SkinRev Lab, CityLink Mall, SKINLIFT Face is S$888 per session for early laxity along the jowls and jawline, SKINTIGHT is S$588 per zone for crepey texture, and the Needleless SkinBooster is S$288 per region (first region from S$98), all before GST.

  • Keep a record of each treatment: product, amount, area and date.
  • Ask for a review before topping up, not a fixed calendar of injections.
  • Compare photographs taken in the same light, not the mirror on a tired day.
  • Prefer small amounts in the right layer over more volume in the same place.
  • If you no longer know what was injected, have it assessed before adding more.

Common questions

Your questions,
answered

How long does lip filler last?

Commonly about six to twelve months. In a meta-analysis of lip studies, 74% of people still had fuller lips at six months and 46% at twelve months. Some filler can remain after the visible effect fades.

How long does cheek filler last?

Often about 12 to 24 months with a firmer volumising gel placed deep. In one randomised trial, nearly half of treated people kept their correction at two years.

How long does under-eye filler last?

Often 12 months or longer, because the area moves little and soft gels can persist. That is also why repeat treatment here is planned cautiously: extra filler can build up and cause puffiness.

Does filler ever go away completely?

HA filler is broken down gradually, but imaging studies show it can remain detectable for years in some people, long after it stops being visible. If you are unsure what is left, an assessment can help.

Why did my filler disappear so fast?

Often it is early swelling settling, which makes the first days look fuller than the real result. A small amount, a soft gel or a mobile area such as the lips also fades from view sooner.

Can filler be dissolved?

HA filler can be dissolved with hyaluronidase, usually over one to three days. Non-HA fillers such as PLLA, PCL and calcium-based fillers cannot be dissolved this way.

Do biostimulators last longer than HA filler?

They can: in one randomised study, PLLA improvement was reported up to 25 months after the last treatment. The change is gradual, needs a course, and cannot be dissolved, so it suits different goals.

How often should I top up filler?

Only when a review shows it is needed, not on a fixed schedule. Because HA can persist longer than it shows, routine top-ups are a common cause of an overfilled look.
