Skin tags (acrochordons) are small, soft growths that hang from the skin, often on a narrow stalk. They are usually skin-coloured or slightly darker, from about 1 mm to a few centimetres, and they gather where skin folds and rubs: the neck, armpits, groin, under the breasts and on the eyelids. They are not contagious and are not a form of cancer. Most people looking for skin tag removal in Singapore want them gone because they catch on necklaces, collars or bra straps, bleed when snagged, or simply look untidy.

No one knows exactly why skin tags form, but friction plays a part, and they become more common with age, weight gain, pregnancy and insulin resistance. That link is useful: several new skin tags in a younger adult can be a nudge to check blood sugar and other metabolic markers. A few lookalikes, such as a raised mole, a seborrhoeic keratosis, a wart or a neurofibroma, can resemble a skin tag, so the first step is for a doctor to confirm what it is.

Assessment by Dr Sin Yong — Dr Sin Yong first confirms that each growth is a skin tag and not a mole, wart or other lesion. Anything that looks atypical is removed in a way that allows histology.

Shave or excision removal by Dr Sin Yong — Small tags on a narrow stalk can be snipped at the base after numbing, with any bleeding stopped by pressure or cautery. Larger or broad-based tags are shaved or excised under local anaesthetic, sometimes with a fine stitch, and can be sent for histology if the diagnosis is not certain.

Radiofrequency/electrosurgical removal — A fine radiofrequency or electrosurgical tip cuts through the stalk and seals small vessels at the same time. It suits clusters of small tags on the neck and gives good control on delicate skin near the eyes.

Cryotherapy — Liquid nitrogen freezes the tag so that it darkens and falls off over the following days to weeks. It needs no cutting, but it can lighten the surrounding skin, which is a consideration on darker and Asian skin.

Skin tags are made of loosely arranged collagen and blood vessels under a thin layer of epidermis, and DermNet notes that they are more numerous in people with obesity and type 2 diabetes, with friction, growth factors and insulin resistance proposed as contributors. The same metabolic link has been seen in children: a retrospective study of young dermatology patients with skin tags found higher rates of obesity, type 2 diabetes and raised blood lipids than in a matched national sample.

On removal, a blinded randomised study comparing cryotherapy with electrosurgery for several common benign growths found that both doctors and patients rated results for skin tags more highly after electrosurgery than after freezing. Pale or depigmented patches occurred in a small number of frozen lesions, and a few electrosurgery sites developed lighter skin or small atrophic scars. Snipping, electrosurgery and freezing are all standard methods; the choice depends on size, site and skin type.

What is the usual method for skin tag removal in Singapore?

Most skin tags are removed by a doctor with a quick snip at the base, a fine electrosurgical or radiofrequency tip, or freezing with liquid nitrogen. Small tags often need only numbing cream or a tiny injection of local anaesthetic. Will skin tags come back after removal?

A tag that is removed at its base does not usually regrow from the same spot. New tags can still appear elsewhere, because the underlying tendency, such as friction, age, weight and insulin resistance, is still there. Keeping skin folds dry, reducing rubbing from tight collars or jewellery and, where relevant, managing weight and blood sugar may help slow new ones.

Can I tie off or cut a skin tag at home?

It is not recommended. Home tying, cutting or freezing can cause bleeding, infection and scarring, and you may be removing something that is not a skin tag at all. The eyelids, groin and anal area are particularly unsuitable for home attempts. A doctor can remove the tag cleanly, control any bleeding and send it for testing if it looks unusual.

Are skin tags a sign of diabetes?

Not on their own, but there is a link. Skin tags are more common in people with higher body weight, insulin resistance and type 2 diabetes. If you notice many new tags, especially with darker, velvety skin on the neck or in the armpits, it is reasonable to ask a doctor about a blood sugar test. Many people with skin tags have normal results.

Is there downtime after skin tag removal?

Very little for most people. Small sites form a tiny scab that falls off within about one to two weeks, and frozen tags darken and drop off over a similar period. You can usually return to work the same day. Keep the area clean, avoid picking, and use sun protection on exposed sites to lower the chance of a dark mark.
