Forehead filler in Singapore uses hyaluronic acid (HA) filler to create a smoother, gently rounded forehead contour where the forehead is flat, sloping, uneven or dented. How it works

The mechanism,

The forehead layers, from the surface down, are skin, subcutaneous fat, the frontalis muscle, the galea (a tough fibrous sheet), a loose areolar layer (the subgaleal space) and the periosteum over the bone. Filler for contouring is usually placed deep, in the subgaleal or supraperiosteal plane, often with a blunt cannula entered at a single point, then spread and moulded to a smooth curve. A soft-to-medium firmness HA gel that spreads evenly is commonly chosen.

Three main arteries supply the forehead: the supraorbital and supratrochlear arteries rising from above the eye socket, and the frontal branch of the superficial temporal artery from the side. Their paths vary, and they travel through different layers as they rise. Mapping them with Doppler ultrasound before treatment, staying in the correct deep plane, using a cannula, injecting slowly and in small amounts, and keeping hyaluronidase ready are the core safety steps.

Forehead filler usually suits adults with a flat, sloping or uneven forehead who want a softer, rounder profile, and people with small dents or depressions in the forehead contour. It can balance the upper face with the nose and chin. It suits people who understand that this is a higher-risk area and are comfortable with a careful, step-by-step approach.

Forehead filler is not advised during pregnancy or breastfeeding, over skin infection, or with a history of severe filler reactions. It is not the right treatment for dynamic forehead lines, which respond better to wrinkle-relaxing injections, or for a heavy brow. People who have had previous forehead filler or surgery in the area need ultrasound assessment first. A very large change in one session raises the risks and should be avoided.

At consultation, Dr Sin Yong assesses your forehead shape from the front and side, asks about previous filler, and explains the risks in plain terms. Where appropriate, vessels are mapped with ultrasound before treatment. On the day, the skin is cleaned thoroughly and a small entry point is numbed. Filler is placed slowly in the deep plane and gently moulded, and the shape is checked with you.

Expect swelling for several days. Because the forehead drains downwards, some puffiness can appear around the eyelids a day or two later and then settles. Avoid pressing or massaging the forehead, wearing tight headbands or caps, strenuous exercise and saunas for a few days. Increasing pain, skin turning pale, blotchy or dusky, or any change in vision is an emergency: contact the clinic straight away.

The evidence,

A review of world literature on blindness from fillers found that the forehead was among the most frequently involved sites, after the nose and glabella, and that once vision loss occurred no treatment was consistently successful. This is why forehead filler is treated as a higher-risk procedure and why prevention, through anatomy knowledge and technique, matters more than any rescue treatment.

In a study of 34 Korean patients having forehead contouring with HA filler, Doppler ultrasound was used to trace the supraorbital, supratrochlear and frontal branch of the superficial temporal arteries in each layer before injection, with an average of 2.85 mL injected. All three vessels were successfully identified and avoided, and no arterial complications occurred. Asian expert recommendations similarly support ultrasound-guided HA placement in the forehead and temple, where vessel anatomy varies.

Why is my eyelid puffy a day after forehead filler?

Swelling from the forehead can drift downwards with gravity and settle around the upper eyelids for a few days. This is common and usually resolves on its own. However, eyelid drooping, double vision, blurred vision or pain around the eye are not normal and need urgent assessment, so contact the clinic straight away if you notice them.

How long does forehead filler last?

It varies by product and area. The deep forehead plane is relatively protected, and HA can persist in tissue longer than it is visible. Rather than setting a fixed repeat interval, your forehead is reviewed at each visit and only treated again if it is needed.

Can forehead filler be dissolved?

Yes, HA filler can be broken down with the enzyme hyaluronidase. Ultrasound can help locate the filler precisely so the enzyme goes where it is needed. Dissolving can be done for shape concerns, lumps or migration, and urgently in the rare event of a vascular problem.
