Cystic acne treatment why deep, painful pimples need a doctor, and what actually helps
Cystic acne is the deep, painful kind that lingers for weeks and leaves scars. Here is what it is, why squeezing makes it worse, when to see a doctor quickly, which prescription treatments are used, and what facials can and cannot do.

Quick answer
- Cystic acne treatment usually needs a doctor, because cystic acne is a severe form of acne with deep, painful lumps (nodules and cysts) that rarely respond to over-the-counter products alone. Doctors combine prescription creams with oral antibiotics, hormonal treatment for some women, or isotretinoin for severe or scarring acne.
- Do not squeeze a cystic pimple: the inflammation is deep, and squeezing pushes it deeper and raises the risk of scars and dark marks. In a study of 1,972 people seeing dermatologists, 43% already had acne scars, and the delay before effective treatment was a key risk factor that can be changed.
- For a single painful cyst, a doctor can inject a weak dilution of triamcinolone, which often flattens it within two to three days, but it does not stop new ones forming. Facials such as HydraFacial can clear blackheads and congestion; they cannot treat cysts, and inflamed lumps are not extracted.
- Prescription medicines and injections are given by Dr Sin Yong at IN Eternity Clinic, Pacific Plaza, after a consultation and when appropriate, priced at consultation. SkinRev Lab does not prescribe or inject.
The basics
What is cystic acne?
Most pimples are small and close to the surface: blackheads, whiteheads, red bumps and pus spots. In cystic acne, some spots form deep in the skin as firm, swollen, tender lumps that can take weeks to settle. Doctors often call these nodules, and the condition nodular or nodulocystic acne; 'cystic acne' is the everyday name.
The lumps form when a blocked follicle becomes so inflamed that its wall gives way deep in the dermis, and the body mounts a strong reaction around it. Because this happens deep down, the lumps usually have no head to drain, and they are the type of acne most likely to leave scars.
Cystic acne is common on the jawline, cheeks, chest, shoulders and back. It affects teenagers and young adults, and also many adult women, whose flares often follow the menstrual cycle along the jaw and chin. A single cystic pimple can also appear in people whose acne is otherwise mild.
Look-alikes
Cystic pimple or something else?
Several other lumps are mistaken for cystic acne. They need different treatment, which is one reason to have deep or unusual lumps checked by a doctor.
| Lump | What it usually looks like | What it needs |
|---|---|---|
| Cystic acne nodule | Deep, tender, red lump, with other acne nearby | Acne treatment by a doctor; sometimes an injection |
| Ordinary pimple or pus spot | Small, close to the surface, settles within days | Gentle skincare; topical treatment |
| Sebaceous (epidermoid) cyst | Slow, painless, movable lump, often with a central pore; may refill | Assessment; removal if troublesome |
| Boil (infected follicle) | Hot, very painful, enlarging lump, sometimes with fever | Prompt medical review; may need draining |
| Fungal acne | Itchy, small bumps all the same size; no blackheads | Antifungal treatment after diagnosis |
| Lumps in the armpits or groin | Painful lumps that keep returning in skin folds | A doctor; this is a different condition |
Hands off
Why you should not squeeze a cystic pimple
A cystic pimple has no plug near the surface to release. Squeezing, needling or extracting it presses the inflamed contents sideways and deeper into the skin, which spreads the inflammation, makes the lump last longer and raises the risk of infection, dark marks and scars.
Picking and squeezing are recognised scar risk factors. When an international group of acne experts built a tool to predict who is at higher risk of atrophic acne scars, lesion manipulation was one of only four factors, alongside the worst-ever severity of acne, how long it had lasted and a family history of acne scars.
- Leave the lump alone, however tempting.
- Keep skincare gentle; skip scrubs and harsh spot treatments on the lump.
- Do not try to drain it with a needle or have it extracted at a salon.
- If it is very painful, or you have an event in a few days, see a doctor about an injection.
- If the lump is hot or spreading, or you feel unwell, see a doctor promptly.
Doctor first
When to see a doctor quickly
Cystic acne is where waiting costs the most. In a study of 1,972 people seeing 120 dermatologists in the United States, 43% already had acne scars. Most of them had only mild or moderate acne at that visit, and the time between acne starting and the first effective treatment was a key risk factor that can be changed.
A GP can start treatment for many people, and a dermatologist manages severe, scarring or stubborn acne. See a doctor if you have any of the following.
- Any deep, painful lumps, even if there are only a few
- Acne that is leaving dents, raised scars or dark marks
- Acne on the chest, back or shoulders that keeps coming back
- No improvement after two to three months of good over-the-counter care
- Sudden adult acne with irregular periods, extra hair growth or weight change, which may need a hormonal check
- Acne affecting your mood, confidence or daily life
- A hot, spreading, very painful lump or a fever, which needs prompt review
Treatments
Cystic acne treatment options a doctor may use
The American Academy of Dermatology's 2024 guideline, built on a systematic review, gives its strongest recommendations to benzoyl peroxide, topical retinoids, topical antibiotics and oral doxycycline, and strongly recommends oral isotretinoin for acne that is severe, scarring, causing distress or not responding to standard treatment. It advises combining treatments that work in different ways and limiting how long oral antibiotics are used. Most treatments need eight to twelve weeks before they can be judged.
| Option (generic name) | What it does | Notes |
|---|---|---|
| Topical retinoids (adapalene, tretinoin) | Unblock pores and prevent new spots | The base of most plans; can irritate at first; avoided in pregnancy |
| Benzoyl peroxide | Reduces acne bacteria and inflammation | Often combined with a retinoid or topical antibiotic; can bleach fabric |
| Topical clindamycin | Antibacterial and anti-inflammatory | Used with benzoyl peroxide to limit resistance |
| Oral doxycycline or minocycline | Reduce inflammation in moderate to severe acne | A limited course, combined with topical treatment |
| Combined oral contraceptive pill or spironolactone | Reduce hormone-driven oil production in women | For suitable women, after medical assessment |
| Oral isotretinoin | Shrinks oil glands; long remission for many | For severe, scarring or resistant acne; needs pregnancy prevention and monitoring |
| Triamcinolone injection | Calms one large inflamed lump | A rescue for single lumps, not a treatment for acne overall |
Isotretinoin
Isotretinoin for severe or scarring cystic acne
Isotretinoin is an oral retinoid that shrinks the oil glands and acts on all the main drivers of acne. Guidelines reserve it for severe, nodular, scarring or stubborn acne, and many people have long remissions after a course lasting several months.
It needs careful supervision. It can cause birth defects, so anyone who could become pregnant must avoid pregnancy during treatment and for a period afterwards, as their doctor directs. Dry lips and skin are very common, and the doctor also discusses mood, blood tests and other effects. Waxing, exfoliating facials, peels and lasers are usually postponed during treatment and for a time afterwards; the timing is decided individually.
A 2018 Cochrane review found most isotretinoin trials were of low quality; compared with antibiotics plus creams, it slightly improved doctor-rated severity, with more minor side effects. The evidence has gaps even for a treatment doctors rely on for severe acne. In Singapore, isotretinoin is a prescription-only medicine.
Hormones
Hormonal cystic acne in adult women
Deep, tender spots along the jaw and chin that flare before periods are a common pattern in adult women. Hormones increase oil production, so treatments that reduce their effect on the oil glands can help. The AAD guideline makes conditional recommendations for combined oral contraceptive pills and spironolactone.
In the SAFA trial of 410 women in England and Wales, spironolactone improved acne-related quality of life compared with placebo, and at 24 weeks 82% vs 63% reported improvement. Headaches were more common with spironolactone, and no serious adverse reactions were reported. It is not used for men, and women taking it should avoid pregnancy.
If acne starts suddenly in adulthood alongside irregular periods, extra hair growth or weight gain, a doctor may check for a hormonal condition such as polycystic ovary syndrome before choosing treatment.
Rescue
A cortisone injection for a single cystic pimple
For one large, painful lump, a doctor can inject a few drops of diluted triamcinolone directly into it. In a survey of 100 dermatology practitioners, the most common concentration was 2.5 mg/mL and the most common volume about 0.05 mL per lesion. The pain often eases within a day and the lump flattens within two to three days.
The trade-off is a small risk of a dent or a paler patch at the injection site. Most practitioners surveyed said fewer than 1% of their patients returned with a side effect, but 48% said a dent, when it happened, lasted more than six months. The AAD guideline lists these injections as an add-on for larger lesions, not a substitute for treating the acne, because they do nothing to stop the next lump.
Facials
What facials can and cannot do for cystic acne
Facials work on the surface. HydraFacial uses a mild glycolic and salicylic acid step and vortex suction to lift out blackheads and congestion, and a small 12-week study of 20 adults with mild to moderate acne found more had clear or almost-clear skin after six treatments, although it had no comparison group. That makes it useful for congestion, but it does not reach a deep nodule or change the oil glands and hormones behind cystic acne.
At SkinRev Lab, painful, cystic or nodular acne is referred to a doctor first, and inflamed lumps are not extracted. HydraFacial can support a doctor's plan for blackheads and whiteheads once the skin has settled, and SKINLDM (S$288 before GST, first visit S$68) is a calming ultrasound facial for hot, reactive skin. If you are taking or have recently stopped isotretinoin, exfoliating facials are postponed until your doctor says it is safe.
Where to go
Cystic acne care in our network
At IN Eternity Clinic, 9 Scotts Road, #12-01 Pacific Plaza, beside Orchard MRT, Dr Sin Yong grades the acne, rules out look-alikes such as fungal acne, cysts and boils, and, when appropriate after a consultation, prescribes treatment or injects a painful lump. Once the acne is under control, he can assess marks and scars as a separate step, for example for 4D Scar Reconstruction. Treatment is priced at consultation by the clinic; WhatsApp +65 8815 3008.
SkinRev Lab, at CityLink Mall #B1-28, linked underground to City Hall and Esplanade MRT, does not prescribe or inject. HydraFacial Syndeo (S$288 to S$438 before GST, first-timer rate at the counter) and SKINLDM can support skin care alongside a doctor's treatment; WhatsApp +65 8266 9585.

Common questions
Your questions,
answered
How do you get rid of a cystic pimple fast?
Leave it alone and see a doctor if it is large or painful. A doctor can inject a weak dilution of triamcinolone into the lump, which often flattens it within two to three days. Squeezing or needling it at home usually makes it larger, longer-lasting and more likely to scar.
Should I pop a cystic pimple?
No. A cystic pimple sits deep in the skin with no head to drain, so squeezing pushes the inflammation deeper and raises the risk of infection, dark marks and scars.
How long does a cystic pimple last?
Untreated, a deep acne lump can take a few weeks to settle, and it may leave a red or brown mark for months. An injection by a doctor can shorten that to days, and treating the acne itself reduces how often new lumps appear.
What causes cystic acne in adults?
The same processes as teenage acne: extra oil, blocked follicles, acne bacteria and inflammation, which in cystic acne runs deep. Hormones play a large part for many adult women, and the tendency often runs in families. Heavy, occlusive products, friction and some medicines can add to it.
Do I need antibiotics or isotretinoin for cystic acne?
Often one or the other, depending on severity. Oral antibiotics such as doxycycline are combined with creams for a limited time, and isotretinoin is recommended for severe, scarring or stubborn acne. A doctor decides after examining the skin, and both are prescription-only.
Is cystic acne hormonal?
Often, especially in adult women with flares along the jaw and chin around their periods. Combined oral contraceptive pills or spironolactone can help suitable women. Sudden adult acne with irregular periods or extra hair growth should be checked for a hormonal cause.
Can a facial or HydraFacial treat cystic acne?
No. Facials can clear blackheads and congestion and calm the skin, but they cannot reach deep nodules or change the oil glands and hormones behind them. Inflamed cysts are not extracted, and cystic acne needs a doctor first.
Will cystic acne leave scars?
It is the type of acne most likely to. Deep inflammation, picking and a long delay before effective treatment all raise the risk. Treating early is one of the most useful things you can do, and established scars can be treated once the acne is controlled.
How much does cystic acne treatment cost in Singapore?
It depends on the assessment and the plan, including any medicines, injections and follow-up. At IN Eternity Clinic, Dr Sin Yong's acne treatment is priced at consultation. At SkinRev Lab, HydraFacial Syndeo is S$288 to S$438 before GST, as support for congestion rather than treatment for cysts.
