Eye bags are the puffy bulges under the lower lids that make people look tired even after a good night's sleep. Some are worse in the morning and settle by lunch. Others stay all day and cast a shadow underneath. The most common cause of a constant, all-day bag in adults is orbital fat. The eye sits in a cushion of fat held back by a thin membrane, and imaging studies show this fat increases in volume with age and pushes forward. An MRI study of 40 adults found that the soft tissue below and in front of the eye increases with age, and that expansion of orbital fat was the largest contributor, rather than the eyeball sinking or the fat simply slipping forward. This explains why non-surgical treatments can disguise but not remove a true fat bag. A small randomised split-face study in 15 Chinese adults found that both microneedle fractional radiofrequency and a 1,565 nm fractional laser reduced measures of lower-lid bulging and improved skin over three monthly sessions.

For the groove beneath the bag, a 2024 meta-analysis of 31 reports including 2,556 people found high satisfaction with hyaluronic acid tear trough filler. Swelling and bruising were each seen in about one in five, with lumps and a bluish tint less common. A systematic review of delayed complications found that swelling was the most frequent late problem, appearing on average more than a year after injection. That is why careful patient selection matters around the eyes.

How can I tell if my eye bags are fat or fluid?

Fluid puffiness usually changes through the day, worse on waking or after salty food, alcohol or crying, and better by afternoon. Fat bags are present all the time and often bulge more when you look upward. Many people have a mix. If one eye swells more, or swelling is sudden or painful, see a doctor to rule out allergy, infection or other medical causes.

Is tear trough filler a good idea for eye bags?

It suits some people, not all. Filler tends to work better when the main problem is a hollow groove under a small bulge and the skin is not very thin. In people who are prone to fluid puffiness, filler can attract water and make swelling worse, sometimes months later. How long is the downtime?
