Pillow face, also called facial overfilled syndrome, is a common reason people look for pillow face correction in Singapore. It describes a face that looks swollen, heavy or unnaturally rounded after too much filler, or after filler placed in the wrong layer. Common signs include cheeks that bulge forward, a loss of the natural curve between the lower eyelid and cheek, puffiness that is worse in the morning, a wide or heavy lower face, and expressions that look stiff or distorted when smiling.

Pillow face correction is the process of finding where the excess hyaluronic acid (HA) filler sits and dissolving it in a targeted way. How it works

The mechanism,
in plain English

Overfilling usually builds up slowly. HA filler can persist in tissue much longer than many people expect, so repeated treatments build up in layers. HA also binds water, which adds to the swelling. Filler placed too superficially, or under pressure from facial muscles, can drift and change shape. As the face ages beneath it, the same amount of filler can sit differently.

Correction starts with high-frequency ultrasound, which shows HA filler as dark (anechoic) pockets in the tissue and reveals the layer, size and position of each deposit, along with nearby vessels. Hyaluronidase is then injected directly into the deposits that need reducing, rather than spread across the whole face. The enzyme breaks the bonds in the HA chains so the body can clear them, usually over a day to a few days. Targeting matters because filler can be surrounded by tissue that slows the enzyme's spread. Hyaluronidase has no effect on non-HA fillers, so knowing what material is present comes first.

Pillow face correction usually suits people who feel their face looks heavy, wide, puffy or unlike themselves after fillers, people with morning puffiness or bulges in previously filled areas, and those who have had filler in several places over several years and are unsure what is still there. It also suits people who want to reset before deciding whether any future filler is needed.

Hyaluronidase only works on HA filler. It will not dissolve poly-L-lactic acid, calcium hydroxylapatite, polycaprolactone, silicone or other non-dissolvable fillers, or fat grafts; these need a different plan. People with a history of allergy to hyaluronidase, or significant allergy to bee or wasp stings, need careful assessment and testing first. It is not advised during pregnancy or with active infection. If puffiness is from fluid retention or an underlying medical cause, that needs to be addressed separately.

What to expect

At consultation, Dr Sin Yong reviews your filler history and looks at your face at rest and in motion. An ultrasound scan maps where filler sits, which helps decide what to dissolve and what to leave. Your allergy history is checked, and a small test may be done. On the day, the skin is cleaned and numbing cream may be applied. Hyaluronidase is placed into the mapped deposits through a fine needle or cannula, guided by ultrasound.

Swelling, redness, itch or tenderness can last a day or two, and some areas may briefly look hollower while tissue rebalances. Avoid massaging the area, strenuous exercise and saunas for about a day. Your face is reviewed a few weeks later, once it has settled, to see what remains. Some people need further rounds spaced weeks apart, and any new filler is considered only after the face has fully settled. A course is usually planned at consultation.

What the research says

The evidence,

Clinical reviews describe facial overfilled syndrome as a growing complication driven by anatomical mismatch, repeated layering of filler and the way facial muscles and tissue move around it. A 2026 narrative review concluded that targeted, ultrasound-guided hyaluronidase is the established approach for HA filler, while other adjuncts need caution, and that prevention depends on anatomy knowledge and individualised planning. An earlier anatomical review noted that the syndrome is reported more often in East Asian faces, and that MRI and ultrasound studies challenge the idea that filler breaks down quickly.

MRI follow-up of HA in the midface found filler present in all 33 patients studied at least two years after injection, and in some for many years, supporting a conservative approach to repeat treatment. Ultrasound literature shows that previous filler can be seen before new treatment, that displaced filler and complications can be visualised, and that hyaluronidase can be injected directly into the deposit under ultrasound guidance.

Why use ultrasound for pillow face correction in Singapore?

Ultrasound shows where HA filler sits, how much there is and which layer it is in, as well as nearby vessels. This lets the enzyme be placed directly into the excess, rather than spread widely and dissolving filler you might want to keep. It also helps identify fillers that are not HA, which hyaluronidase cannot dissolve.

Will dissolving filler make my face saggy?

Some people notice a brief hollow or flatter look for a few days as swelling settles and tissue rebalances. Very occasionally, skin that was stretched by a lot of filler may look a little looser afterwards. In most cases, natural contours return. If structure is genuinely missing once the excess is gone, small, careful amounts can be considered later.

Is hyaluronidase safe?

Can all filler be dissolved?

Only hyaluronic acid filler can be dissolved with hyaluronidase. Collagen-stimulating fillers such as poly-L-lactic acid and calcium hydroxylapatite, and non-dissolvable materials such as silicone, do not respond. If you do not know what was used before, ultrasound can sometimes help identify the material, and the plan is adjusted accordingly.

When can I have filler again after dissolving?

Wait until the face has fully settled, usually a few weeks after the last round, and review it with fresh eyes. Many people find they need far less than before, or none at all. If structure is missing, it is better to rebuild with small amounts in the right layer than to return to the pattern that caused the overfilling.
