Tear trough filler in Singapore uses a small amount of soft hyaluronic acid (HA) filler to soften the groove that runs from the inner corner of the eye down and across the upper cheek. When this groove deepens, it casts a shadow that looks like a dark circle and makes people look tired even after a good night's sleep. It is one of the most common reasons people ask about under-eye treatment.

Not every dark or tired under-eye is a tear trough. Shadows can come from a hollow, from pigment in the skin, from visible blood vessels under thin skin, or from fat bulging forward (eye bags). How it works

The mechanism,
in plain English

The tear trough sits where the thin eyelid skin meets the thicker cheek skin, over a ligament that tethers the tissue to the bone of the eye socket rim. A soft, low-swelling HA gel is placed in small amounts deep on the bone (supraperiosteal), under the orbicularis oculi muscle, either with a needle or with a blunt cannula entered from the cheek. Total volumes are small, often a fraction of a millilitre per side, because the area swells easily and the skin is thin.

Placement too shallow can leave a bluish tint (the Tyndall effect) or visible lumps, and the lower eyelid area is prone to delayed swelling because of how it drains lymph fluid. The angular and infraorbital vessels run nearby and connect with the circulation of the eye, so anatomy knowledge, slow injection and conservative amounts matter. Ultrasound can help check for old filler and vessel position where useful. Supporting the cheek first sometimes improves the tear trough with less filler near the eye.

Is it right for you

Who it suits,
and who it does not

Tear trough filler usually suits adults with a visible hollow groove under the eye, reasonably firm skin with good elasticity, little or no fat bulging forward, and no tendency to fluid retention or puffiness. It suits people whose 'dark circles' are mainly a shadow from the hollow rather than pigment, and who are happy with a subtle, gradual approach.

Tear trough filler is not a good choice for puffy eye bags from fat bulging forward, which usually need surgical assessment, or for people with frequent morning puffiness, allergies or fluid retention, as filler can make swelling worse. Very thin or crepey skin may show filler, and pigment-based dark circles respond better to laser or skincare. It is not advised during pregnancy or breastfeeding, with eye or skin infection, or soon after eye surgery.

At consultation, Dr Sin Yong examines your under-eyes in different light and angles, checks for fat bulging, skin quality, pigment and puffiness, and asks about allergies and previous filler. He will tell you honestly if filler is not the right tool. If you go ahead, the area is cleaned, numbing cream is applied, and very small amounts are placed, checking as he goes.

Afterwards, expect swelling and possibly bruising, which can take up to a week or so to fade in this thin-skinned area. Sleep with your head slightly raised for a night or two, avoid rubbing your eyes, and avoid saunas, strenuous exercise and alcohol for a day or two. Any vision change, eye pain, or pale or dusky skin is an emergency. Delayed swelling weeks or months later should also be reviewed.

What the research says

The evidence,

A 2024 meta-analysis of 31 reports with 2,556 participants found high satisfaction after HA injection of the tear trough, with a low rate of serious problems. The common side effects were swelling (about 19%) and bruising (about 18%), with redness, contour irregularity or lumps, and a blue tint (Tyndall effect) less often. A separate meta-analysis of prospective studies found that needle injection was associated with more bruising, while cannula injection was associated with more swelling, so the technique choice involves trade-offs.

Delayed problems also happen. A systematic review of 52 cases of delayed complications after tear trough filler found swelling was the most common, followed by lumps or nodules, a xanthelasma-like yellowish reaction, migration and discolouration, appearing on average more than a year after injection. Most delayed swelling cases involved HA. The authors stressed that clinicians should understand these risks, recognise them promptly and inform patients beforehand.

How much does tear trough filler in Singapore cost?

Will tear trough filler get rid of my dark circles?

Only if the darkness is a shadow from a hollow groove. Pigment in the skin, visible vessels under thin skin, and puffy fat bags look similar but have different causes. Pigment usually responds to laser and skincare, thin skin to boosters, and bulging fat to surgery. Dr Sin Yong will explain which you have before suggesting anything.

Is tear trough filler in Singapore safe?

When done conservatively by an experienced doctor, it is generally safe, but it is a delicate area. Bruising and swelling are common. Less commonly, lumps, a bluish tint or delayed swelling occur, sometimes many months later. The vessels near the eye connect to the eye's circulation, so vascular events, though rare, are possible. HA can be dissolved with hyaluronidase if problems arise.

Needle or cannula for tear troughs?

Both are used. Studies show needles tend to cause more bruising, while cannulas tend to cause more swelling. A needle allows precise deep placement on the bone; a cannula can treat a longer area through fewer entry points. Dr Sin Yong chooses based on your anatomy and explains why.

Why is my under-eye puffy weeks after filler?

The lower eyelid area drains fluid slowly, and HA attracts water, so some people develop swelling that appears or persists weeks or months later, especially if too much filler was used or it sits too high. This can usually be improved with hyaluronidase. Please get it reviewed rather than waiting, especially if there is redness or pain.

How long does tear trough filler last?

It varies by product and area. The under-eye moves less than the lips and soft gels can persist for a long time, sometimes longer than expected. Because extra filler here can build up and cause puffiness, repeat treatment is planned cautiously and only when it is needed.
