Dark underarms are very common and usually harmless, but they can make people self-conscious in sleeveless tops or at the gym. The skin may look evenly darker, patchy, greyish from hair shadow, or slightly thickened. Underarm skin is a distinct environment. A review of axillary skin describes how routine shaving and plucking damage the skin, causing redness and dryness in the short term and, in some people, post-inflammatory hyperpigmentation over time (Evans et al.). Acanthosis nigricans, by contrast, appears as symmetric, velvety, darker plaques in skin folds such as the underarms and neck; a review describes its link with insulin resistance and notes that management begins with weight reduction and treating any underlying cause (Das et al.).
A 2025 systematic review of treatments for axillary hyperpigmentation found only ten studies. Q-switched Nd:YAG laser was effective across three studies without severe side effects, intense pulsed light also helped, and several topical agents showed varying degrees of improvement, but there is still no standard treatment (Vahabi et al.). The evidence base is small, so expectations should be realistic and treatment should follow the cause rather than a fixed recipe. In practice, that means reducing irritation and friction first, then adding brightening care, and keeping laser for darkening that persists despite those changes.

Does laser for dark underarms hurt?
