After a pimple heals, it often leaves a flat brown or dark mark that lingers for weeks or months. This is post-inflammatory hyperpigmentation, or PIH: pigment cells overreact to inflammation and leave extra melanin behind. It is not a scar, because the surface is flat and the texture is normal. Acne-related PIH is common in Asia. A survey by the Asian Acne Board of 324 people attending dermatologists for acne in seven Asian countries found that more than half had PIH, that marks often lasted a year or longer, and that many found them as bothersome as the acne itself; picking was common and is a habit that can be changed (Abad-Casintahan et al.). A review of PIH in skin of colour recommends treating the underlying inflammation early, with daily sun protection and topical lightening agents first, and keeping procedures such as laser for resistant cases, used with caution (Davis and Callender).

A 2024 systematic review of 41 studies and 877 people found that complete clearance of PIH was uncommon with any method, although most people achieved partial improvement with lasers, topical agents or combinations, and a small number worsened after laser or energy-based treatment (Kashetsky et al.). A focused review of acne and procedure-related PIH in Asians makes the same point: lasers can help as an add-on or when topicals fail, but the treatment itself can worsen PIH if it is too aggressive (Eimpunth et al.).
