Thread veins, also called spider veins or broken capillaries, are fine red, purple or bluish lines just under the skin. On the face they usually appear on the sides of the nose, the cheeks and the chin, sometimes with a background of general redness. On the legs they often form branching webs on the thighs and around the knees and ankles. Anyone researching thread veins treatment in Singapore should know that facial vessels and leg vessels behave differently, and are usually treated differently too.

The medical name is telangiectasia: small vessels near the surface that have widened and stayed that way. On the face, the usual causes are cumulative sun damage, rosacea, genetics and ageing, with hormonal change, alcohol, long-term use of strong prescription creams on the face and skin injury contributing in some people. On the legs, long hours standing, pregnancy, family history and pressure in the deeper veins play a bigger part. Thread veins themselves are harmless, but many people find them hard to cover.

A systematic review of 152 randomised studies in rosacea, graded with the GRADE method, found low-to-moderate certainty evidence that laser and intense pulsed light reduce redness and, mainly, visible vessels. In a comparison on the nose, both a long-pulsed 1064 nm Nd:YAG laser and a pulsed dye laser improved rosacea-related vessels to a similar degree, but the Nd:YAG laser did better on thick, dilated vessels, while fine vessels within background redness responded better to the pulsed dye laser. Vessel type therefore guides the choice of device and settings.

For leg veins, a systematic review of 26 studies covering 1,991 patients found long-pulsed 1064 nm Nd:YAG laser a safe option for leg vessels up to 3 mm across, with clearance similar to injection treatment of the veins (sclerotherapy). Sclerotherapy was usually less painful and quicker to show change, and combining approaches gave better clearance in some studies. These findings come from long-pulsed settings, which differ from low-energy toning, so Dr Sin Yong tells you plainly when another approach or referral suits your vessels better. Will my thread veins come back after treatment?

Does laser for thread veins hurt?

Most people feel a quick snapping or warm sensation with each pulse, and cooling is used to keep it tolerable. Afterwards the area may look red or slightly swollen for a few hours to a couple of days, and occasionally there is mild bruising. Avoid hot showers, saunas and strenuous exercise for a short time after treatment, and protect the area from sun.

How can I prevent new thread veins on my face?

Wear a broad-spectrum sunscreen every day, including on cloudy days and in the car, and add a hat in strong sun. Treat rosacea if you have it, wash with lukewarm rather than hot water, avoid harsh scrubbing and pore strips, and keep alcohol moderate if it makes you flush. These steps do not remove existing vessels but help limit new ones.
