How it works

The mechanism,
in plain English

Lifting threads are made from absorbable surgical-suture polymers and usually carry small barbs or cogs along their length. They are passed through a few fine entry points using a blunt cannula or needle and laid in the fat layer just under the skin. When the tissue is gently moved upwards over the barbs, they grip it and hold it in its new position. Placing the threads along vertical vectors aims to work against the direction in which the face has dropped, rather than pulling sideways towards the ears, which can look flat or wide.

Over the following months the thread material is broken down and resorbed — within about a year, depending on the polymer. As this happens, a mild foreign-body response forms collagen-rich tissue around the thread track, which helps support the area as the thread itself disappears.

The energy-based step heats the dermis and support layers to tighten skin and stimulate further collagen. Combining mechanical repositioning with thermal tightening is the reason the protocol is described as multi-modal.

Is it right for you

Who it suits,
and who it does not

RVR ProLift usually suits adults with mild to moderate sagging who want more visible repositioning than energy treatments alone can give, but who are not ready for, or do not need, surgery. Typical concerns are early jowls, a descended mid-face, a jawline that has lost its edge and mild asymmetry between the two sides. Reasonable skin quality and moderate soft-tissue thickness help, because the threads need tissue to grip and should not be visible through very thin skin. It also suits people who accept a short recovery and understand that results are gradual and need maintenance.

RVR ProLift is not suitable during pregnancy or breastfeeding, with active skin infection or acne in the area, with uncontrolled diabetes or a bleeding disorder, or for people on blood thinners that cannot be paused safely under medical advice. Tell the doctor about autoimmune conditions, keloid scarring, recent dental infection, previous threads, non-absorbable implants or fillers in the face. Very thin skin with little fat may show or feel threads. Heavy sagging with large folds of loose skin is better served by a surgical opinion, and mainly volume loss is better addressed with biostimulators or filler.

A consultation with Dr Sin Yong comes first. He analyses how the face has descended, where the vectors should run, whether energy should be part of the plan and whether other treatments would serve you better. Photographs, and imaging where needed, are taken for planning and later comparison. Fees, risks and aftercare are explained before anything is booked. A course is usually planned at consultation, and Dr Sin Yong reviews symmetry at the end of each session to decide whether staged work is needed.

On the day, numbing cream is applied, then local anaesthetic at the entry points and along each vector. Threads are placed through a small number of fine entry points, and the energy step follows if planned. Afterwards, expect mild swelling, tightness and small entry marks for two to three days, sometimes bruising. Avoid wide mouth opening, heavy chewing, facial massage, strenuous exercise and saunas for the period you are advised, and sleep on your back for the first few nights.

What the research says

The evidence,

The thread-lift literature needs careful reading. A 2018 systematic review of barbed thread-lift sutures found that, over the preceding decade, little robust evidence had been added on efficacy or safety, and that most studies showed limited durability of the lift; the two strongly positive studies were company-sponsored. A 2021 systematic review covering 59 studies and over 14,000 treated people found that the most common side effects were asymmetry, swelling and bruising, mostly temporary, while serious problems such as nerve or gland injury were rare.

Combining threads with energy is a newer idea with early supporting data. A 2025 retrospective study of 58 people treated with barbed threads plus focused ultrasound on the mid and lower face reported improvements in physician-rated scores and in jowl fat-pad position across sessions, with treatment order not affecting the outcome. This is why RVR ProLift is planned conservatively: realistic goals, careful vector placement and honest discussion of how long the effect may hold.
