Daily sun protection is the foundation. In a randomised study of 903 adults in Australia, people assigned to daily broad-spectrum sunscreen showed no detectable increase in skin ageing over 4.5 years and aged less than those who used sunscreen only when they chose to (Hughes et al.). In a comparison of women living in a heavily polluted city and a less polluted city in China, long-term pollution exposure was linked with more severe pigment and skin-structure signs, and a panel of non-expert women judged the faces from the polluted city as duller and less radiant (Flament et al.). Weather and UV conditions in the two cities were similar, which points to pollution as an added factor.
For lasers, a systematic review of picosecond lasers graded the published evidence for pigment and rejuvenation and concluded that they are a safe and effective option for benign pigmented lesions and photodamage when used appropriately (Wu et al.). Low-fluence 1064 nm laser toning is widely used in Asia, but a case series of 14 women documented mottled loss of pigment after repeated toning, sometimes after only a few sessions, which is why settings and frequency should be decided by a doctor (Chan et al.).
