How it works

The mechanism,

Facial muscles contract when nerves release acetylcholine at the junction between nerve and muscle. A neuromodulator temporarily blocks that release in the small area where it is injected, so the muscle contracts less strongly. The skin above it is folded less, and the line it creates softens. Nearby muscles that are not injected keep working, which is how a natural range of expression is preserved.

The effect usually starts within a few days and builds to its full level over about two weeks. Over the following months, the nerve endings form new connections and muscle activity gradually returns, so treatment is repeated if you want to maintain the effect. Many people find their lines are less deep over time, because the skin is creased less often.

Modern practice favours neuromodulation rather than freezing: lower doses spread thoughtfully, matched to the strength of each muscle and to how it interacts with its neighbours. He explains what will soften, what will not and how much movement to expect afterwards. Photographs are taken at rest and in motion for comparison. A course is usually planned at consultation, with a review about two weeks after the first treatment.

The injections take a few minutes. A very fine needle places small amounts into each target muscle; most people feel brief pricks. Small bumps at the injection points usually flatten within 15 to 30 minutes, and occasional pinpoint bruises can be covered with make-up. For the rest of the day, avoid rubbing or massaging the treated areas, lying flat for a few hours, saunas and strenuous exercise. The effect appears over a few days and is reviewed at about two weeks.

Wrinkle-relaxing injections are among the most studied aesthetic treatments. A meta-analysis of seven randomised, double-blind, placebo-controlled studies with 1,474 people found that a single standard dose was far more effective than placebo at improving frown lines, including their appearance at rest, with no significant difference in adverse events between treatment and placebo groups.

A global consensus of plastic surgeons and dermatologists, published in 2016, recommended an assessment-led, patient-tailored approach: identifying when muscle activity is the main cause, using lower doses in the upper face, and combining treatment with hyaluronic acid filler where volume loss contributes. A narrative review of complications identified brow drop and asymmetry as the commonest unwanted effects of these injections, which is why careful assessment of brow position and accurate placement matter. Results vary between people, and most side effects are temporary.
