At low power, red laser light does not burn tissue. Instead, it is absorbed by an enzyme in the mitochondria — the energy-producing parts of cells — called cytochrome c oxidase. This is thought to raise cellular energy output, release small amounts of nitric oxide and improve local blood flow around the follicle. The main target is the dermal papilla, the cluster of signalling cells at the base of each hair that controls how long a hair keeps growing.
In pattern hair loss and chronic shedding, too many follicles drift into the resting (telogen) phase, and the growth (anagen) phase gets shorter. Low-level light appears to nudge resting follicles back into growth and help them stay there longer, so hairs come through thicker and are shed less often. This only works on follicles that are still alive. A smooth, shiny bald patch has no active follicles left to stimulate, so no amount of light will change it.

H2LT is not a treatment for areas that are already bald and smooth, and it is not suitable for scarring alopecias, where follicles are destroyed by inflammation. An itchy, flaky or inflamed scalp, such as seborrhoeic dermatitis, is usually settled first, and any scalp infection is treated before starting. Baseline photographs are taken from fixed angles. During a session you sit comfortably with protective eyewear while the laser is applied across the thinning areas. There is no numbing cream. Most people feel nothing, or a faint warmth or tingle. The strongest single study of low-level laser for hair is a multicentre, randomised, double-blind study comparing a laser device with an identical-looking sham device in men and women with pattern hair loss. Over 26 weeks, terminal hair density increased significantly more in the laser groups than in the sham groups, regardless of age or sex, and no serious adverse events were reported. A 2017 systematic review of 11 studies found that most showed improved hair counts or density, although patient-rated satisfaction was more modest than the measured changes.
A 2025 meta-analysis pooling 38 studies confirmed a significant increase in hair density with low-level laser or LED therapy compared with placebo in androgenetic alopecia, but found too little data to draw conclusions for other types of hair loss. One honest caveat: most published studies used home devices, so evidence specific to in-clinic protocols such as H2LT laser Singapore patients receive is more limited.
