Prevention has strong support. In a randomised study of 903 adults in Australia, people assigned to daily broad-spectrum sunscreen showed no detectable increase in skin ageing over 4.5 years and aged less than those who used sunscreen only when they chose to (Hughes et al.). Because the neck and hands receive daily sun, applying sunscreen there is a simple step with evidence behind it.

For treatment, a systematic review of 46 studies on rejuvenating the back of the hand, covering fat grafting, injectable fillers and laser or light treatments, found that most studies reported good results with mainly mild side effects, and that satisfaction was lower with laser and light treatments than with volume replacement (Ovadia et al.). For the neck, a prospective study of hyaluronic acid filler placed with a blunt cannula into horizontal neck lines found that improvement peaked at one month and declined by three months, with some benefit still present at six months after one session (Rongthong et al.). Expectations about the duration of neck results should therefore be modest, and the plan should include maintenance.

Why do the backs of my hands look veiny and bony?
