Fat grafting correction in Singapore: when a fat transfer leaves the face heavy, lumpy or uneven
Grafted fat is living tissue, so it cannot be dissolved like filler. This guide explains why a facial fat transfer can end up too full, lumpy or lopsided, how a doctor works out what is actually there, and the realistic options, from waiting and doing nothing to surgery by a plastic surgeon.

Quick answer
- Fat grafting correction means assessing and, where it is worthwhile, reducing or smoothing grafted fat that has left the face fuller, heavier, lumpier or more uneven than planned. It starts with working out what is fat, what is filler and what is ageing, because each is handled differently.
- Grafted fat that survives becomes living tissue with its own blood supply. Hyaluronidase, the enzyme used to dissolve hyaluronic acid (HA) filler, has no effect on it, and grafted fat can grow if you gain weight.
- Options run from waiting and no treatment, through doctor-led measures such as a carefully dosed steroid injection for an inflamed nodule, and energy-based lipo-remodelling of excess graft near the surface, to microliposuction or excision by a plastic surgeon. None is right for every face, and results vary.
- Dr Sin Yong assesses fat graft problems at SkinRev Lab, CityLink Mall, and IN Eternity Clinic, Pacific Plaza, and treatment is priced at consultation. He does not perform fat grafting or liposuction; when surgery is the better route, he refers you to a plastic surgeon.
The basics
What fat grafting correction means
Facial fat grafting, also called fat transfer or lipofilling, is surgery. A surgeon harvests fat by liposuction, usually from the abdomen or thighs, processes it and injects it in small parcels into the cheeks, temples, forehead, under-eyes, jawline or lips. Only part of the fat survives, and how much survives varies between people and between areas of the face.
Fat that survives picks up a blood supply and becomes part of you. That is what makes a good result long-lasting, and it is also what makes an unwanted result hard to undo: there is no enzyme or injection that simply dissolves grafted fat the way hyaluronidase dissolves HA filler.
Fat grafting correction is therefore not one procedure. It is an assessment of what has gone wrong and why, followed by a choice between doing nothing, treating a specific problem such as an inflamed lump, gently reducing excess graft near the surface, or removing fat surgically. Sometimes the right answer is simply to wait.
Causes
Why a fat transfer ends up too full, lumpy or uneven
Because some grafted fat is always lost, surgeons often place a little more than the final volume they want. When more of it takes than expected, the face ends up fuller than planned. A review of facial fat grafting complications notes that overcorrection is seen more often after repeated grafting sessions than after a single session, because each area's survival is hard to predict and the face carries the total.
Weight gain adds a later layer. Grafted fat keeps the character of the place it came from, so fat taken from the tummy or thighs can grow when you gain weight. A systematic review of six reports found exaggerated, out-of-place growth of facial grafts after weight gain, which can appear months or years after a result that looked balanced.
Lumps and unevenness come from how the graft behaved. In a 2022 systematic review of 298 adverse events after facial fat grafting, irregularities made up about 19%, graft hypertrophy (overgrowth) about 7%, fat necrosis about 7% and asymmetry about 4%. The authors stressed that the true complication rate is unknown because reporting is inconsistent.
Ageing continues underneath. Bone, ligaments and skin change over the years, so grafted fat can sit lower or look heavier than it did at first, and any filler added later can build up over it.
What it looks like
Common problems after facial fat grafting, area by area
People usually describe what they see rather than what is underneath, so it helps to know what each problem usually looks like. The same appearance can come from different causes.
Fullness that is worse when you smile, a face that looks wider than before, or a puffy area that does not change through the day all suggest excess volume. A firm or tender bump suggests a nodule, which needs a closer look.
| Area | What people notice | Often confused with |
|---|---|---|
| Cheeks | A rounder, wider face, heaviest when smiling; loss of the cheekbone line | Filler build-up, weight gain |
| Under the eyes | A ridge, bulge or visible lump below the lower lid; puffiness that does not settle | Eye bags, fluid, under-eye filler |
| Temples and forehead | A domed or heavy upper face; a visible edge where the graft ends | Brow descent, filler |
| Beside the mouth and jawline | Bulk that blurs the lower-face outline | Jowls, sagging |
| Any grafted area | A lump, uneven edge or one side fuller than the other | Fat necrosis, oil cyst, filler nodule |
Assessment
Fat, filler or both? Finding out first
Many people who have had fat grafting have also had filler, before, after or in the same areas. On the surface they can look alike, but they are corrected in completely different ways. Hyaluronidase breaks down HA filler only; it does nothing to fat. Dissolving filler from a face whose fullness is really graft achieves little, and adding filler to balance a heavy graft usually makes things worse.
Assessment starts with a full history: which areas were grafted, how many sessions, when, where the fat came from, whether any filler or collagen stimulator was added, and how your weight has changed since. The face is examined at rest and while moving.
Ultrasound helps show what sits where. A 2026 review of filler imaging describes material-specific patterns: HA tends to appear as dark, fluid-like pockets, while grafted fat appears as soft-tissue nodules that may show changes of fat necrosis. Where HA filler is also present, it is usually dealt with first and the graft reassessed afterwards.
Timing
When to wait, and when to be seen promptly
Swelling after fat grafting takes weeks to settle, and the graft keeps establishing itself, and losing volume, over the following months. Judging the result too early risks treating swelling, or fat that would have been reabsorbed anyway. Correction is generally considered only once the result has been stable for some time; your surgeon can advise on timing for your case.
Gradual heaviness is rarely urgent. Increasing pain, redness, warmth, a discharge, fever, or a hard, tender lump that is growing is different and should be seen promptly by your surgeon or a doctor, because infection, inflammation and fat necrosis are among the causes.
Any change in vision, eye pain, weakness of the face or arm, or difficulty speaking after a facial injection or graft is an emergency. Go to an emergency department at once.
Options
The options for fat grafting correction, compared
The choice depends on how much excess there is, how deep it sits, whether there is a discrete lump, whether the skin is thin, and what you want. Including no treatment as an option matters: a small irregularity that only you notice may be better left alone, and some changes soften as swelling settles.
Published options include intralesional steroid for inflamed nodules, small doses of triamcinolone or deoxycholic acid for tiny irregularities, liposuction and direct excision. Each acts on tissue, and each can overshoot, leaving a dent or thin skin, so they are used sparingly and in small, tested steps.
| Option | What it can do | What it cannot do | Who |
|---|---|---|---|
| Wait and review | Let swelling settle and the graft stabilise; avoid treating what will change anyway | Remove a stable excess | You, with your surgeon or doctor |
| Steroid injection | Calm an inflamed fat necrosis nodule or granuloma | Reduce broad fullness; overuse can thin skin or cause a dent | A doctor, sparingly |
| Energy-based lipo-remodelling (Time Freeze Laser LCLR®) | Gradually reduce excess graft near the surface over a series | Remove a large or deep excess, or a hard lump, in one step | Dr Sin Yong |
| Microliposuction | Remove fat through fine cannulas | Avoid surgical risks such as dents, irregularity and nerve irritation | An accredited surgeon |
| Excision | Remove a discrete, firm lump or cyst | Treat diffuse fullness; it leaves a scar | A plastic or oculoplastic surgeon |
| Hyaluronidase | Dissolve HA filler sitting alongside the graft | Dissolve fat; it has no effect on grafted fat | A doctor, after assessment |
Non-surgical
Time Freeze Laser LCLR® for excess graft near the surface
Time Freeze Laser LCLR® (Laser Collagen Lipo Remodelling) is Dr Sin Yong's own protocol on a long-pulsed 755 nm and 1064 nm laser platform. One part heats the dermis to tighten skin; the other, a sculpting mode, applies controlled bulk heating to superficial fat in zones he selects. On his website, Dr Sin Yong describes using this lipo-remodelling arm where a fat graft has been overfilled, has moved, or has left uneven areas.
Zones are chosen from the examination and ultrasound, the depth and pass pattern are set for each area, and any excess is reduced gradually over a planned series rather than in one session, with review in between so that only the excess is treated. Treated zones may feel warm, tender or slightly swollen for a day or two.
It is important to be clear about the limits. LCLR is a clinic protocol, and no published study has tested it on fat grafts. Change is gradual and varies between people. It is not a substitute for surgery when the excess is large, deep or has formed a hard lump, and it is not used while a graft is still settling or over infected or inflamed skin. Where lifting or contour is also needed, Dr Sin Yong may add VF Lift, his monopolar radiofrequency protocol.
Surgery
Microliposuction and excision by a plastic surgeon
When the excess is large, well defined and stable, or a non-surgical series has not been enough, removing fat surgically may be the better route. Microliposuction uses fine cannulas to take fat out; excision cuts out a discrete lump or cyst. In a 2026 series of 38 women who had microliposuction for an overfilled forehead after fat grafting, the forehead contour flattened measurably and satisfaction was high; every patient had temporary upper-eyelid swelling, and two had short-lived nerve symptoms that resolved.
Results are not always complete. In a study of temple overgrowth after fat grafting, 31 people answered the satisfaction survey after liposuction: about 23% rated the result satisfactory and about 45% largely satisfactory, and the authors noted the procedure was not easy and could not correct every case.
Microliposuction is a form of liposuction, and in Singapore liposuction is regulated by the Ministry of Health. It may be performed only by doctors accredited under the liposuction framework, in a hospital or an approved ambulatory surgical centre, after a cooling-off period of at least seven days. Fat grafting itself is also surgery and is performed by plastic surgeons; excision near the eyes is usually done by an oculoplastic surgeon.
At SkinRev Lab
What happens when you see Dr Sin Yong
Dr Sin Yong is the doctor at SkinRev Lab, which is licensed by the Ministry of Health as SkinRev Clinic. Your consultation is with him: he takes the history of grafting, filler and weight change, examines the face at rest and in movement, and uses ultrasound where it helps to separate graft, filler and ageing. His approach to an over-treated face is to find out what is already there before adding anything, treat only the layer that has changed, and always include no treatment as an option.
You then get a plain account of what he thinks is going on and the options that fit, which may be waiting, dissolving any HA filler that is part of the problem, Time Freeze Laser LCLR® for excess graft near the surface, or referral to a plastic or oculoplastic surgeon when surgery is the better tool. Treatment is priced at consultation, and nothing is done on the day unless you are ready.
Consultations are at SkinRev Lab, #B1-28 CityLink Mall, 1 Raffles Link, with an underground link to City Hall and Esplanade MRT, open daily 11am to 8pm (WhatsApp +65 8266 9585), or at IN Eternity Clinic, 9 Scotts Road, #12-01 Pacific Plaza, beside Orchard MRT (WhatsApp SkinRev Lab on +65 8266 9585 or IN Eternity Clinic on +65 8815 3008). No facial or non-invasive treatment at SkinRev Lab reduces grafted fat, and we will tell you so.

Common questions
Your questions,
answered
What is fat grafting correction?
It is the assessment and, where worthwhile, treatment of a face that a fat transfer has left fuller, lumpier or more uneven than planned. It starts by working out what is fat, what is filler and what is ageing, and then considers options from waiting to surgery.
Can grafted fat be dissolved like filler?
No. Hyaluronidase dissolves hyaluronic acid filler only. Grafted fat that has survived is living tissue, so it has to be reduced by methods that act on fat, or removed surgically.
How long should I wait after fat grafting before correction?
Until the result has settled and been stable for some time. Swelling takes weeks to go down and the graft keeps changing over several months, so your surgeon's advice on timing matters. A painful, red or growing lump should be seen sooner.
Why does my face look fuller years after fat grafting?
Grafted fat behaves like the fat it came from and can grow with weight gain. Ageing, and any filler added later, can also make the same areas look heavier, so each is assessed.
Can an overfilled fat graft be corrected without surgery?
Sometimes, when the excess is modest and near the surface. Dr Sin Yong uses the lipo-remodelling arm of Time Freeze Laser LCLR® over a planned series in suitable cases. Change is gradual and varies, and a large, deep or hard excess is usually better treated surgically.
Does SkinRev Lab perform fat grafting or liposuction?
No. Fat grafting and liposuction are surgery, performed by surgeons. Dr Sin Yong assesses problems after fat grafting and treats suitable cases without surgery, and refers to a plastic or oculoplastic surgeon when surgery is the right tool.
Who should I see for a lump after fat grafting?
Start with the surgeon who did the graft if you can, especially in the first months. A second opinion from a doctor who can examine and scan the area is reasonable if you are unsure what the lump is. A lump that is growing, painful or changing should be checked promptly.
Will adding filler balance out an uneven fat graft?
Usually not when the problem is too much volume. Adding filler to a heavy area tends to add to the problem. Filler is occasionally used for a small hollow next to a graft, but reduction and assessment come first.
How much does fat grafting correction cost?
It depends on what the problem is, how many areas are involved and which approach is chosen. At SkinRev Lab and IN Eternity Clinic, treatment by Dr Sin Yong is priced at consultation, after the assessment.
