Buccal fat removal in Singapore what the surgery does, its long-term risk, and non-surgical alternatives

Buccal fat removal promises sculpted cheeks, but it is permanent surgery on fat that thins with age anyway. Here is who it may suit, what the research cannot yet tell you, and how to slim a full face by treating its real cause.

Quick answer

  • Buccal fat removal in Singapore is a surgical procedure, usually done through a small cut inside the cheek, in which a surgeon removes part of the buccal fat pad, a deep pocket of fat between the cheek muscles, so the lower cheeks look less full. It is permanent, and there is little published follow-up of how faces look years afterwards.
  • Reviews report high short-term satisfaction and mostly minor complications, about 4% across 921 procedures in one review. Surgeons warn that facial fat thins naturally with age, so removing it young may leave the cheeks looking hollow or gaunt later.
  • Many full faces are not caused by buccal fat at all. A wide jaw is often a large chewing muscle, fullness under the chin is submental fat, and a heavy lower face can be early sagging; each has its own non-surgical option.
  • No facial or massage can melt the buccal fat pad, which sits beneath the cheek muscles. SkinRev Lab does not perform surgery or injections; it offers SKINLIFT® and SKINSHAPE face zones, while masseter injections and fat freezing are performed by Dr Sin Yong at IN Eternity Clinic and priced at consultation.

The basics

What is buccal fat, and what does removal do?

The buccal fat pad is a rounded pocket of fat deep in the cheek, lying between the chewing and cheek muscles rather than just under the skin. Anatomical dissections describe three lobes held in place by small ligaments, with extensions that reach up towards the temple and back behind the jaw. It acts as a gliding cushion for the muscles you use to chew and make expressions, and its volume changes over a lifetime.

In buccal fat removal, also called buccal lipectomy or buccal fat pad reduction, a surgeon makes a small incision inside the mouth near the upper back teeth, eases out part of the pad and closes the wound with dissolving stitches. There is no outside scar. It may be done under local anaesthesia, with sedation, or alongside a facelift.

The aim is a less rounded lower cheek and a more visible cheekbone contour. It does not change the jaw, the chin or fat under the chin, and it does not tighten skin.

Evidence

What the research says about buccal fat removal

The evidence is thinner than social media suggests. A 2021 systematic review found only four eligible studies: satisfaction was high, 7 complications were reported across 134 cheek procedures, and the authors noted poor methods and a lack of long-term follow-up.

A 2026 systematic review of ten studies with 1,123 patients again found high satisfaction and a 4.2% complication rate, mostly minor, but long-term outcomes were poorly assessed; one study reported a gaunt appearance in 1.6%. An earlier analysis titled 'Proceed with caution' found no published long-term follow-up at all, and warned that the effect of age-related fat loss and late deformities had simply not been studied.

In short, buccal fat removal is usually safe in the short term in trained hands, but nobody can yet say with confidence how a face will look 10 or 20 years later.

Long-term risk

The long-term hollowing risk

Facial fat does not stay the same. It sits in separate compartments that change at different rates, and a CT study of 60 Asian women found some mid-face compartments thinning with age while others thickened. As these changes happen, cheeks that looked full at 25 can look flatter at 45 without any surgery.

Removing buccal fat takes away part of that reserve for good. A result that looks sculpted in your twenties may look hollow or gaunt once natural fat loss, weight loss or endurance sport is added on top. Volume can be restored later with fillers or collagen stimulators, but these add support rather than recreate the original pad.

This is why many surgeons are conservative about who they offer it to and how much they remove.

Who suits it

Who may suit buccal fat removal, and who should not

Buccal fat removal is surgery, performed by a plastic, oral and maxillofacial, or ear, nose and throat surgeon. SkinRev Lab does not perform surgery.

  • May suit: adults with a stable weight and genuinely full lower cheeks from a prominent buccal pad, confirmed on examination, with good skin quality.
  • Usually not suited: faces that are already slim or long, high cheekbones with little fat beneath them, or early hollowing under the cheekbones.
  • Think twice if you plan to lose weight, do a lot of endurance sport, or are past your mid-thirties, when natural fat loss is already under way.
  • Not the answer for a wide jaw from the masseter, fullness under the chin, jowls from sagging, or a round face caused by steroid medicines or a hormone condition.
  • Ask the surgeon how much will be removed, what happens if you lose weight later, and how hollowing would be managed if it appeared.

Find the cause

Why your face looks full: matching cause to option

Many faces have more than one cause, and a slim face can still have a strong jaw or fat under the chin. An examination of muscle, fat, skin and bone comes before any slimming plan.

Jaw muscle

The masseter: a cause buccal fat removal cannot fix

A square or wide lower face is often the masseter, the thick chewing muscle at the angle of the jaw. A prominent masseter is common in East Asian faces and can be enlarged by clenching and grinding. Removing buccal fat does nothing to it.

Masseter injections, a wrinkle-relaxing injection placed into the muscle, make it work less so it gradually thins, usually visibly from two to four weeks. A 2026 meta-analysis of five placebo-controlled trials with 536 people found short-term reductions in muscle thickness and prominence, without an agreed ideal dose. The effect is temporary, and side effects include tired chewing and, occasionally, an uneven smile.

At IN Eternity Clinic, Dr Sin Yong examines the jaw at rest and while clenching before treating, and the treatment is priced at consultation. SkinRev Lab does not inject, and no device at SkinRev Lab slims the masseter.

Under the chin and sagging

Double chin and loose skin: non-surgical face slimming

Fat under the chin sits just beneath the skin, the layer non-invasive treatments reach. At SkinRev Lab, the SKINSHAPE double chin or lower face zone uses microwave energy on pinchable fullness (S$288 per zone per session, first zone S$88, before GST), and SKINLIFT® Double Chin (S$488 per session before GST) lifts and firms the area under the chin. At IN Eternity Clinic, fat freezing with a small applicator is performed by Dr Sin Yong; in a study of 60 people, cryolipolysis under the chin reduced the fat layer by about 2 mm on ultrasound.

When the fullness is mainly early sagging, lifting the jawline changes the outline more than removing fat. SKINLIFT® Face (S$888 per session before GST) suits early jowls. VF Lift, performed by Dr Sin Yong at SkinRev Lab, and HIFU or threads at IN Eternity Clinic suit more established laxity and are priced at consultation.

Honest note

Can facials or massage melt buccal fat?

No. The buccal fat pad lies deep, beneath the cheek muscles and close to the inside of the mouth. No facial, cream, gua sha, roller or massage reaches it or reduces it, and buccal massage done inside the mouth works on muscles, not on the fat pad. A lymphatic facial or massage can reduce puffiness for a day or so, which is why some people see a temporary change.

Energy devices act mainly on fat just under the skin, not on the deep pad. Losing weight shrinks facial fat along with fat elsewhere, but you cannot choose where it goes first. If a treatment promises to melt buccal fat without surgery, ask how it reaches fat that sits under muscle.

Common questions

Your questions,
answered

How much does buccal fat removal cost in Singapore?

It is surgery, so the fee is set by the surgeon and usually covers the surgeon, anaesthesia and facility. SkinRev Lab does not perform it. For non-surgical slimming, SKINSHAPE face zones are S$288 per zone and SKINLIFT® Double Chin S$488 per session before GST; medical options at IN Eternity Clinic are priced at consultation.

Is buccal fat removal permanent?

Yes. The fat that is removed does not grow back, although the remaining fat still changes with weight and age. That is the main concern: as facial fat thins naturally over the years, the cheeks can end up looking hollower than they otherwise would.

Will buccal fat removal make me look older?

It might, years later, in some faces. Published studies have very little long-term follow-up; one review reported a gaunt look in 1.6%. Faces that are already slim, and people past their mid-thirties, are generally considered at higher risk of hollowing.

Who is a good candidate for buccal fat removal?

Adults with a stable weight and genuinely full lower cheeks from a prominent buccal pad, confirmed on examination, with good skin quality. It does not suit faces that are already slim or long, or fullness caused by the jaw muscle, fat under the chin or sagging.

What is recovery like after buccal fat removal?

Expect swelling that settles over one to a few weeks, a soft diet and careful mouth hygiene while the inside wound heals, and a final contour that can take a few months to show. Your surgeon will give specific aftercare.

Can I get rid of buccal fat without surgery?

Not the buccal fat pad itself, because no facial, massage or device reaches it. You can often slim the face by treating other causes: the jaw muscle, fat under the chin, sagging or puffiness. Overall weight loss reduces facial fat, but not selectively.

Buccal fat removal or masseter injections: which do I need?

Clench your teeth: if the jaw angle bulges, the muscle is the main cause and injections can help. If the fullness sits below the cheekbone and does not change when you clench, buccal fat may be part of it. A doctor's examination tells them apart.

Does buccal fat go away with age?

Facial fat generally thins and shifts with age, and the buccal pad's volume changes over a lifetime, though this varies. Fullness that bothers you at 22 may soften naturally, which is one reason removing it early raises concern about hollowing later.

Does SkinRev Lab do buccal fat removal?

No. SkinRev Lab is a non-invasive centre and does not perform surgery or injections. It offers SKINLIFT® and SKINSHAPE for fullness under the chin and early laxity, and passes enquiries about masseter injections or fat freezing to IN Eternity Clinic.
