How to remove fat transfer from face: realistic options, their limits and who to see

Fat transfer is often described as long-lasting, which is welcome until the result is not what you wanted. Grafted fat is living tissue, so it cannot be dissolved the way hyaluronic acid filler can. Here is what can realistically be done, by whom, and what each approach cannot do.

Quick answer

  • You cannot remove fat transfer from face tissue with an injection that dissolves it. Grafted fat that has survived is living tissue with its own blood supply; hyaluronidase, the enzyme that dissolves hyaluronic acid (HA) filler, has no effect on it.
  • Surgical options are microliposuction, which removes fat through fine cannulas, and excision, which cuts out a discrete lump or cyst. Both are done by plastic or oculoplastic surgeons, can improve contour, and carry risks such as dents, irregularity, bruising and nerve irritation. Results are not always complete.
  • Non-surgical options are more limited: a carefully dosed steroid injection for an inflamed nodule, tiny doses of other agents for small irregularities, and, for modest excess near the surface, gradual reduction with an energy-based protocol such as Time Freeze Laser LCLR®. Keeping your weight stable stops the graft growing.
  • Removal is usually partial and aimed at balance, not at getting back exactly to before. Dr Sin Yong assesses what is there at SkinRev Lab and IN Eternity Clinic, treats suitable cases without surgery, and refers when surgery is the better route; treatment is priced at consultation.

Why it is different

Why you cannot simply dissolve a fat transfer

HA filler is a manufactured gel. Hyaluronidase breaks down the hyaluronic acid chains so the body can clear them, usually over a day or a few days, which is why unwanted filler can be reversed. Guidance on hyaluronidase in aesthetic medicine describes its use for HA filler problems; it is a prescription medicine given by a doctor after assessment.

Grafted fat is not a product. Once it has picked up a blood supply, it is part of your own fat layer, woven between the fat that was already there. No enzyme selectively removes it, and hyaluronidase does nothing to it.

Drugs that destroy fat cells, such as deoxycholic acid, act on any fat they reach, cause swelling and can harm nearby structures. They have been described only in tiny doses for small irregularities after grafting, not as a way to reverse a fat transfer.

Before deciding

What removal can realistically achieve

Total removal is rarely possible or wise. Grafted fat is spread in many tiny parcels and mixed with your natural fat, so taking it all out would also remove fat you need. Faces lose fat and bone with age, and a face that is reduced too far now can look hollow later.

A more useful goal is balance: reducing the areas that are too full, smoothing lumps or ridges, and evening out one side against the other. It helps to point to specific areas in photographs rather than asking for the graft to be removed in general.

Timing matters. Swelling settles over weeks and graft volume can keep falling for months, so decisions are generally made only once the result has been stable for some time. An exception is a lump that is red, hot, painful or growing, which should be seen promptly.

Surgery

Microliposuction to remove fat transfer from face areas

Microliposuction removes fat through small incisions using fine cannulas. In a 2026 series of 38 women treated for an overfilled forehead after fat grafting, using MRI to plan the layers, the forehead flattened measurably and satisfaction was high; every patient had temporary upper-eyelid swelling and two had short-lived nerve symptoms that resolved.

It is not always enough. In a study of temple overgrowth after fat grafting, about 23% of the patients surveyed after liposuction rated the result satisfactory and about 45% largely satisfactory; the authors described the procedure as difficult and unable to correct every case. Risks include dents, irregularity, bruising, swelling and nerve irritation, and more than one session may be needed.

In Singapore, liposuction is regulated by the Ministry of Health. It may be performed only by doctors accredited by the Accreditation Committee on Liposuction, in a hospital or an approved ambulatory surgical centre, after a cooling-off period of at least seven days, and each repeat procedure has its own cooling-off period.

Surgery

Excision of lumps, cysts and calcified graft

When the problem is a discrete lump rather than general fullness, a surgeon can cut it out. Excision suits a persistent fat necrosis nodule, an oil cyst that keeps returning after drainage, or a calcified area, and has been used to remove grafted fat that grew with weight gain.

Excision leaves a scar, which surgeons place where it is least visible, and it does not help diffuse fullness. Around the eyes it is usually done by an oculoplastic surgeon, through the eyelid or the inside of the lower lid where possible.

After any surgical revision, swelling takes weeks to settle and the contour is judged later, so plan for follow-up visits. Ask the surgeon whether tissue removed will be sent for examination, which can confirm what a lump was.

Injections

Steroid and other injections: small, targeted roles

For an inflamed fat necrosis nodule or lipogranuloma, corticosteroid, as tablets or as small injections into the nodule, is the treatment reported as successful in the largest number of published cases. It calms inflammation and can shrink a nodule; it does not remove a healthy graft.

A review of facial fat grafting complications describes triamcinolone or deoxycholic acid injections for micro-irregularities, starting with test doses and increasing gradually. Both act on tissue indiscriminately: too much, or too shallow, can thin the skin, leave a dent or change colour. They are for small areas, used sparingly, and are not a substitute for surgery for larger excess. These are prescription medicines given by a doctor after assessment.

Non-surgical

Energy-based reduction of excess graft near the surface

Dr Sin Yong uses the lipo-remodelling arm of Time Freeze Laser LCLR®, his own protocol on a long-pulsed 755 nm and 1064 nm laser platform, to reduce excess graft that sits near the surface. Zones are chosen after examination and ultrasound, and the excess is reduced gradually over a planned series, with review between sessions so that only the excess is treated.

Its limits are important. It is a clinic protocol with no published studies on fat grafts; change is gradual and varies between people; it does not remove a large or deep excess, a hard lump or a cyst; and it is not used while a graft is still settling or over infected or inflamed skin. Treated zones may feel warm, tender or slightly swollen for a day or two. Where lift or contour is also needed, he may add VF Lift, his monopolar radiofrequency protocol.

Weight

Weight, and fat transfer that has grown

Grafted fat keeps the character of where it came from, and a systematic review found reports of facial grafts growing out of proportion after weight gain. If your face has become heavier alongside weight gain, part of the change may be graft hypertrophy.

Keeping weight stable prevents further growth. A gradual return towards your earlier weight may help, but how much grafted fat shrinks with weight loss has not been well studied, and weight loss can also hollow areas that were not grafted. This is worth discussing before any procedure, because surgery planned at one weight may look different at another.

Who to see

Who to see about removing fat transfer

The surgeon who performed the graft is usually the first port of call, particularly within the first year. Microliposuction and excision are done by plastic surgeons, and around the eyes by oculoplastic surgeons. Ask how often they perform revisions, what they expect to improve, and what could go wrong.

An aesthetic doctor can help with assessment, separating graft from filler, dissolving HA filler that is part of the problem, and non-surgical options for suitable cases, and should tell you plainly when surgery is the better route. A lump with unusual features may need a dermatologist, imaging or a biopsy.

At SkinRev Lab

Assessment with Dr Sin Yong

At SkinRev Lab, licensed by the Ministry of Health as SkinRev Clinic, your consultation is with Dr Sin Yong. He takes the history of grafting, filler and weight change, examines the face at rest and in movement, and uses ultrasound where it helps to show what is graft and what is filler. His approach is to find out what is already there before doing anything, to treat only the layer that has changed, and to include no treatment as an option.

He will explain which of the options above fits, including whether HA filler should be dissolved first, whether Time Freeze Laser LCLR® is suitable, or whether you should see a plastic or oculoplastic surgeon. He does not perform fat grafting, liposuction or excision. Treatment is priced at consultation.

See him at SkinRev Lab, #B1-28 CityLink Mall, 1 Raffles Link, linked underground to City Hall and Esplanade MRT, open daily 11am to 8pm (WhatsApp +65 8266 9585), or at IN Eternity Clinic, 9 Scotts Road, #12-01 Pacific Plaza, beside Orchard MRT (WhatsApp SkinRev Lab on +65 8266 9585 or IN Eternity Clinic on +65 8815 3008).

Common questions

Your questions,
answered

Can fat transfer to the face be reversed?

Not in the way HA filler can. Grafted fat that has survived is living tissue. It can be reduced surgically by microliposuction or excision, and modest excess near the surface can sometimes be reduced gradually without surgery, but removal is usually partial.

Can hyaluronidase dissolve fat transfer?

No. Hyaluronidase acts only on hyaluronic acid filler. It helps only if some of the fullness is HA filler, which an examination and ultrasound can help identify.

Can fat dissolving injections remove a fat graft?

Not as a way to reverse a graft. Deoxycholic acid has been described only in tiny doses for small irregularities after grafting, and it can cause swelling, dents and nerve problems. It is not among the treatments offered at SkinRev Lab.

Is microliposuction safe for removing facial fat transfer?

It is a recognised revision and published series report improvement with high satisfaction, but risks include dents, irregularity, bruising and temporary nerve symptoms. In Singapore it must be done by an accredited doctor in a hospital or approved surgical centre.

How soon can I have a fat transfer removed?

Usually only once the graft has settled and been stable for some time, as swelling and volume keep changing for months. A painful, red or growing lump is the exception and should be seen promptly.

Will removing fat make my face look hollow?

It can if too much is taken, now or as you age. That is why the aim is usually to reduce specific areas to restore balance rather than remove the graft entirely.

Can laser remove fat transfer from the face?

Not completely. Dr Sin Yong uses Time Freeze Laser LCLR® to reduce modest excess graft near the surface gradually in suitable cases; it does not remove deep excess, hard lumps or cysts, and results vary.

Will losing weight remove my fat transfer?

It may reduce a graft that grew with weight gain, but this has not been well studied and weight loss affects the whole face. Keeping weight stable prevents further growth.

How much does it cost to remove fat transfer from the face?

It depends on the approach. Surgical fees are set by the surgeon and facility and include anaesthesia and follow-up. Non-surgical treatment by Dr Sin Yong at SkinRev Lab or IN Eternity Clinic is priced at consultation, after the assessment.

Who removes fat transfer from the face in Singapore?

Plastic surgeons, and oculoplastic surgeons around the eyes, perform microliposuction and excision. Dr Sin Yong assesses the face, treats suitable cases without surgery and refers when surgery is the better route.
