Trapezius slimming in Singapore trap injections, posture, and what really changes the neck-to-shoulder line

A bulky upper trapezius can make the neck look shorter and the shoulders sloped. Here is why it bulks, how trap injections work, what the small studies show, the risks of weakening a working muscle, and the posture and physiotherapy options.

Quick answer

  • Trapezius slimming usually means relaxing the upper trapezius, the muscle running from the base of the skull and neck to the shoulder, so that it thins and the neck-to-shoulder line looks longer and lower. It is done by a doctor with wrinkle-relaxing injections into the muscle, often called trap injections or shoulder injections.
  • The evidence is small but consistent: in a randomised trial of 20 women and a study of 22 women, the visible muscle area shrank by three months, with the change still present at six months in the second study. The effect is temporary because the muscle recovers, and unlike jaw (masseter) slimming, there are no large placebo-controlled trials.
  • The main trade-off is function. The trapezius lifts and steadies the shoulders, so too strong a dose or misplaced injections can cause shoulder weakness or fatigue, especially when lifting or carrying; a collapsed lung has been reported after a long needle went too deep, and for neck pain a 2026 Cochrane review found only slight, short-term relief.
  • SkinRev Lab does not inject, and trapezius injections are not among the treatments listed for IN Eternity Clinic. Dr Sin Yong can assess whether the bulk is muscle, fat or posture and advise whether an injection suits you; posture-related tension is often better helped by a physiotherapist.

Trapezius slimming: what it is and why people ask for it

The trapezius is a broad, flat muscle across the back of the neck and upper back. Its upper part runs from the base of the skull and the neck down to the outer collarbone and the top of the shoulder blade, and it forms the slope between your neck and shoulder. When it is thick, the neck can look shorter and the shoulders higher and more sloped, especially in photos and in collarless or off-shoulder clothes.

Trapezius slimming, also searched as shoulder slimming, trap injections or shoulder injections for a slim neck, aims to soften that slope. It became popular in East Asia alongside jaw slimming, and most studies come from Korea and China. It is a cosmetic choice about a healthy, working muscle, which is why the risks deserve as much attention as the result.

Why it bulks

Why the upper trapezius bulks: training, posture and tension

The upper trapezius shrugs and lifts the shoulders, helps rotate the shoulder blade when you raise your arms, and steadies the neck. Like any muscle, it grows with heavy, repeated use, and its size is partly inherited.

Not every full neck-to-shoulder line is muscle, so the causes below include the look-alikes.

  • Training: shrugs, deadlifts, farmer's carries, heavy rows and overhead pressing all load the upper trapezius.
  • Posture: a head-forward, round-shouldered position at a desk or over a phone keeps the upper trapezius working to hold up the head and shoulders.
  • Tension and stress: many people hold their shoulders raised without noticing, especially when stressed, cold or concentrating.
  • Load: heavy bags on one shoulder, carrying children, and work that keeps the arms raised.
  • Not muscle at all: fat over the upper back or at the base of the neck, or simply broad, high-set shoulders and collarbones.

Self-check

Muscle, fat or posture? How to tell before any treatment

Only the muscle part responds to injections, so it is worth checking what you are looking at. These checks are a starting point; a doctor examines you standing, at rest and while you shrug, and may use ultrasound to measure the muscle's thickness.

A soft, fatty hump at the base of the neck, especially with a rounder face, easy bruising, new stretch marks or weight gain, can come from steroid medicines or Cushing's syndrome and should be checked by a GP first.

Trap injections

Trap injections: how wrinkle-relaxing injections slim the trapezius

The medicine used in wrinkle-relaxing injections temporarily blocks the nerve signal to the part of the muscle where it is placed. The doctor maps the bulkiest part of the upper trapezius while you shrug and injects small amounts at several points along it, keeping well away from the top of the shoulder. A session takes minutes.

Because the muscle is used less, it slowly thins. In studies the change was already measurable at one month and still present at three and six months; as the nerve signals recover, the muscle regains bulk, so the effect is temporary and would need repeating to keep it.

Placement matters for both the result and safety. Injections too close to the shoulder can weaken the muscles that lift the arm, and deep injections in slim people risk reaching the lung. Doses for a large body muscle are higher than for the face, which is one reason this is a medical procedure for a doctor experienced in it.

Evidence

What the studies show, honestly

The research is promising but thin. In a randomised, double-blind trial of 20 women comparing two doses, the visible area of the trapezius fell significantly by 12 weeks in both groups, with no difference between doses, no complications reported and a median satisfaction score of 4, which the authors described as moderate. In a retrospective study of 22 women, photographs showed the trapezius surface area about 27% to 29% smaller at one, three and six months, ultrasound confirmed a thinner muscle, and no adverse events were recorded.

Compare that with jaw slimming, where a 2026 meta-analysis pooled five placebo-controlled randomised trials with 536 participants. For the trapezius there are no large placebo-controlled trials, follow-up rarely goes beyond six months, and the right dose and injection pattern are not settled. Most participants were young East Asian women, so results may differ in other people.

Many people also hope trap injections will ease neck and shoulder tension. A 2026 Cochrane review of 16 trials with 855 people found that these injections into neck muscles may give only slight-to-small short-term relief of neck pain, about 6 points on a 100-point scale, with no long-term benefit for function and more minor side effects such as injection soreness, muscle fatigue and headache.

Risks

Risks of trapezius injections: weakness, fatigue and rare complications

The trapezius is a working muscle, so the main risks are functional. Too much effect, or medicine reaching nearby muscles, can cause weakness when shrugging, lifting the arms overhead or carrying bags, a tired or heavy feeling across the shoulders, neck ache or stiffness, and uneven shoulders if one side responds more. These effects are temporary but can last weeks to months.

Other effects include pain, bruising and swelling at the injection points and headache. A collapsed lung (pneumothorax) has been reported for this cosmetic use: a woman developed chest pain and breathlessness after a 4.8 cm needle was used where the skin-to-lung distance was only about 2.3 cm, and she recovered over a month. Rarely, the medicine spreads beyond the injected area, causing wider weakness or difficulty swallowing or breathing, which needs urgent care.

Injections are generally avoided in pregnancy and breastfeeding, with neuromuscular conditions such as myasthenia gravis, with infection at the site, and with some interacting antibiotics. Think carefully if your work or sport depends on shoulder strength, such as lifting, swimming, climbing or racket sports, or if you already have shoulder or neck problems.

Without injections

Posture, stretching and physiotherapy for a bulky or tense trapezius

If posture and tension drive the bulk, changing the load is the lasting fix. A physiotherapist can assess how you hold your head and shoulders and set a programme: stretches for the upper trapezius and chest, strengthening for the middle and lower trapezius and the muscles that hold the shoulder blades back and down, and habits that stop the shoulders creeping up.

Be realistic about what that achieves. A 2026 meta-analysis of 28 randomised trials in people with a round-shouldered, head-forward posture found corrective exercises clearly improved posture, but benefits for pain, function and strength were limited and inconsistent. Better posture can make the neck look longer straight away, while any change in muscle size from a lighter load takes months.

  • Set screens at eye level and keep the keyboard close, so your shoulders can drop.
  • Swap a heavy one-shoulder bag for a backpack or a lighter load.
  • If trap size bothers you and you lift weights, ease off shrugs, heavy carries and very heavy pulls for a while.
  • Check in with your shoulders through the day: drop them, breathe out and unclench your jaw.
  • See a doctor for neck pain after an injury, pain that wakes you, or numbness, tingling or weakness in the arms.

How trapezius slimming relates to masseter slimming and neck lines

Trap injections work on the same principle as jaw slimming injections into the masseter: relax a large muscle so that it thins through disuse. The masseter has stronger evidence, with five placebo-controlled randomised trials pooled in a 2026 meta-analysis, and is a smaller muscle; the trapezius is larger, needs higher doses and does more everyday work, so the functional trade-off is greater. Some people want both for a slimmer face-to-shoulder line, but each should be assessed on its own.

Neither treatment changes neck lines. Horizontal tech neck lines are creases in the skin across the front of the neck, treated with skin-quality treatments or fillers, and vertical neck bands come from the platysma, a thin sheet of muscle at the front of the neck. A longer-looking neck from a slimmer trapezius smooths neither.

Expectations

Realistic expectations: what changes and what does not

If the bulk is mainly muscle, a good result is a lower, smoother slope from neck to shoulder and a neck that looks a little longer, appearing over one to three months. Bone width, the height of the collarbones and shoulder blades, fat over the upper back and your posture do not change.

The effect fades as the muscle recovers. Studies have followed people to six months, when the change was still present, and every repeat treatment means repeat exposure to the risks. An international consensus panel has warned that as aesthetic doses approach those used in medical treatment, the body is more likely to form antibodies that stop the injections working, so sensible gaps between sessions, rather than frequent top-ups, are a common precaution.

If fat over the upper back is part of the picture, that is a separate concern: at SkinRev Lab, SKINSHAPE and SKINUSHAPE treat the back and bra line at S$288 per 15 × 15 cm zone per session before GST, with S$88 for a first zone. They reduce fat, not muscle.

Where to go

Trapezius slimming in our network: who does what

SkinRev Lab, at #B1-28 CityLink Mall by City Hall MRT, is a non-invasive aesthetic centre and does not inject. It can help only where upper-back fat, rather than muscle, is the concern, and its team will say so when it cannot help.

Trapezius injections are not among the treatments listed for IN Eternity Clinic, where Dr Sin Yong performs wrinkle-relaxing injections and jaw slimming injections. At 9 Scotts Road, #12-01 Pacific Plaza, beside Orchard MRT, he can examine your neck and shoulders, work out how much is muscle, fat or posture, and advise whether an injection suits you or another route makes more sense; any treatment is priced at consultation. Common questions

Your questions,
answered

What are trap injections?

Trap injections are wrinkle-relaxing injections placed into the upper trapezius, the muscle that slopes from the neck to the shoulder. Relaxing it makes it thin over weeks, so the neck can look longer and the shoulder line lower. They are a prescription medical treatment given by a doctor.

Do trapezius injections really work?

Small studies suggest they reduce muscle bulk: the trapezius area fell by 12 weeks in a randomised trial of 20 women, and by about 27% to 29% on photographs in a study of 22 women. There are no large placebo-controlled trials yet, and fat, posture and bone width do not change.

How long do trap injection results last?

Change can be measured from about a month, and studies found it still present at three and six months; the muscle then gradually regains bulk, so treatment would need repeating to keep the result.

Are trap injections safe?

Side effects are usually mild and temporary, such as soreness, bruising and headache, but shoulder weakness or fatigue can occur, especially when lifting or carrying. A collapsed lung has been reported after a long needle went too deep. Choose a doctor experienced in the procedure, and avoid it in pregnancy or with neuromuscular conditions.

Will slimming my trapezius make my neck look longer?

If the slope is mainly muscle, a thinner upper trapezius lowers the line between neck and shoulder, which makes the neck look longer. If fat, posture or broad, high-set shoulders are the main reason, the change will be small.

Can trap injections help neck and shoulder pain?

Possibly a little, for a short time. A 2026 Cochrane review found that injections into neck muscles may give slight-to-small short-term relief of neck pain, with no long-term benefit for function and more minor side effects. For posture-related pain, physiotherapy is the usual first step.

How can I slim my trapezius naturally?

Reduce what makes it work hard: ease off shrugs and heavy carries, set screens at eye level, use a backpack instead of a one-shoulder bag and keep your shoulders relaxed. Exercises for the middle and lower trapezius improve posture; the muscle itself shrinks only slowly, over months.

Can I still go to the gym after trap injections?

Usually yes, from the next day, but shrugging, overhead and heavy pulling work may feel weaker for some weeks while the effect lasts. Heavy trapezius training also works against the aim of slimming the muscle, so many people ease off it.

How much do trapezius injections cost in Singapore?

It depends on the dose your muscles need and how many sessions you have, and medical injections are priced at consultation after an assessment. SkinRev Lab does not inject. Trapezius injections are not listed among IN Eternity Clinic's treatments, but Dr Sin Yong can assess you and advise whether an injection suits you.
