HIFU side effects and aftercare what is normal, what is not, and how to look after your skin
Most reactions after HIFU are mild and settle within days, but a few need a doctor. Here is what to expect day by day and week by week, the rarer problems worth knowing about, what to avoid and for how long, and the special cases that change the plan.

Quick answer
- The most common HIFU side effects are redness, mild swelling and tenderness: redness usually fades within hours, swelling within about three days, and tenderness to touch within two weeks. Bruising, raised lines (welts), tingling and patchy numbness are less common and usually settle within days to a few weeks.
- Rarer problems include temporary weakness of a facial muscle from nerve inflammation, which usually recovers within two to six weeks, burns or blisters from poor technique, temporary darkening in Asian skin, and loss of fat or volume in a thin face. Eye injury has been reported when energy reached the eye, which is why the eyelids are never treated.
- Aftercare is simple: gentle skincare and daily sunscreen, no saunas, hot yoga or very hot exercise until any swelling settles, and no other treatment on the area for one to two weeks unless your doctor plans them together. Call the clinic about blisters, spreading redness, weakness in a facial muscle, or any eye pain or change in vision.
- In our network, HIFU is performed by Dr Sin Yong at IN Eternity Clinic, where the advice is sunscreen, gentle skincare for a day or two and no saunas or very hot exercise until any swelling settles. SKINLIFT® at SkinRev Lab uses no numbing; most clients return to work straight after, and mild, short-lived redness or slight swelling can occur.
Normal reactions
Common HIFU side effects, and how long they last
Most reactions after HIFU are the expected effects of heat under the skin, and they settle on their own. A review of the safety record of micro-focused ultrasound with visualisation found tenderness, redness and slight swelling to be typical, and bruising, welts and nerve effects to be rare. A 2025 systematic review of 19 studies with 506 patients found swelling, redness and pain after treatment to be the most frequent.
The times below come from the maker's labelling for one widely studied device and from a 2026 review of how complications are managed; your own course may be shorter or longer. In 49 Chinese patients treated with focused ultrasound, bruising appeared in up to a quarter of sessions, so allow for it before an important event.
| Effect | What it looks or feels like | Usually settles within |
|---|---|---|
| Redness | Pink or flushed skin over the treated area | A few hours; occasionally up to two days |
| Swelling | Mild puffiness over the treated area | 3 to 72 hours; up to about a week in some |
| Discomfort and tenderness | Aching after treatment; sore when pressed, especially over bone | Discomfort 2 hours to 2 days; tenderness 2 days to 2 weeks |
| Bruising | Small bruises in the treated area | 2 days to 2 weeks; occasionally about three weeks |
| Welts or raised lines | Lines of raised swelling that match the treatment lines | Often within two days; usually within a week |
| Tingling or numbness | Pins and needles, or a patch of reduced feeling | Typically 2 to 6 weeks |
Rarer risks
Rarer side effects: nerves, burns, pigment and the eyes
Nerve effects. Heat near a facial nerve can inflame it. A motor nerve gives temporary weakness, such as an uneven smile or a brow that will not lift fully; a sensory nerve gives numbness or tingling. The maker reports that these typically resolve within two to six weeks and that no permanent facial nerve injury occurred in its trials, although reports after launch describe nerve effects lasting months. Careful practitioners keep away from set no-go zones over nerves such as the marginal mandibular and supratrochlear nerves.
Burns and scarring. Surface burns, blisters and scars are rare and are linked mainly to technique: a tilted transducer, poor contact with the skin, gel or air pockets, lines placed too close together, or energy stacked in one spot. Rushed treatment raises the risk.
Pigment. Temporary darkening is more likely in darker skin types (Fitzpatrick IV to VI) and with the shallow 1.5 mm depth. In the study of 49 Chinese patients, two developed small dark patches on the forehead that improved over six months, after which the team used a shallower 3.0 mm transducer on the forehead.
Eyes. The maker warns that energy must never reach the eye, which rules out the eyelids and the area inside the bony rim of the eye socket. A reported case of glaucoma and cataract after focused ultrasound around the eyes shows why; eye pain or blurred vision after treatment needs an eye doctor the same day.
Fat loss
Can HIFU cause fat loss or a hollow face?
It can, although published cases are few. A 2025 systematic review found only one case of fat atrophy in the published studies it pooled, but in the US FDA's database of device problem reports, the most frequently reported problems for Ultherapy were fat loss (lipoatrophy), nerve problems and scarring, along with one report of a later facelift being affected. Those reports are unverified and cannot give a rate, but they explain why surgeons and doctors take the concern seriously. The maker's own list of reports since launch includes fat or volume loss, sagging and asymmetry.
The risk is higher in a thin face with little fat, and with over-treatment, because heat at certain depths can affect the fat layer. A careful plan maps fat and skin thickness first and adjusts depth and density by zone. If the main change in your face is volume loss rather than sagging, more heat is the wrong tool, and a filler or biostimulator is usually the better fit.
A related worry is whether HIFU melts filler. The device maker has not evaluated treatment over dermal fillers and advises against treating directly over them, so tell the doctor where and when any filler was placed.
Normal or not
What is normal after HIFU, and when to call the doctor
A useful rule from a 2026 review is time. Mild reactions settle within about 72 hours; reactions lasting from three days to four weeks deserve a check; and anything affecting how a muscle moves, the eyes or the surface of the skin needs urgent attention.
Signs of infection, such as pus or increasing pain and heat, are rare after a treatment that does not break the skin, but they need a doctor promptly.
| Sign | Usually normal | Contact the clinic |
|---|---|---|
| Redness | Fades within hours to two days | Lasts beyond three days, spreads, or blisters or crusts appear |
| Swelling | Mild, easing within a few days | Increases after day three, is hot and painful, or comes with fever |
| Tenderness | Sore to press, easing over two weeks | Severe pain, or pain that keeps worsening |
| Bruising | Small, fading over one to three weeks | Large, growing or very painful |
| Raised lines | Settle within days | Itchy welts still present after a week |
| Numbness or tingling | Patchy, improving week by week | Not improving after several weeks |
| Muscle weakness | Not expected | Any uneven smile, drooping brow or weak lip movement: call the same day |
| Eyes | Not expected | Eye pain, blurred or changed vision: see an eye doctor urgently |
Aftercare
HIFU aftercare, day by day
There is no wound to dress after HIFU, so aftercare is mostly about comfort, heat and sun. The table follows the usual course; your clinic's own instructions come first.
Singapore's heat and humidity make sweating and sun exposure hard to avoid, and the UV index is high all year. Daily broad-spectrum sunscreen and shade matter most while the skin is still pink, because inflamed skin in Asian skin types is prone to darkening.
| When | What to expect | What to do |
|---|---|---|
| Treatment day | Redness, warmth and mild swelling; soreness over bone | Cleanse gently, apply sunscreen and keep skincare simple; skip saunas, hot yoga and very hot exercise |
| Days 1 to 2 | Swelling may peak, then ease; tender to touch | Gentle skincare only; a cool compress can soothe; paracetamol if your doctor agrees |
| Days 3 to 7 | Swelling mostly gone; any welts or bruises fading | Return to your usual skincare once the skin feels normal; keep up sunscreen |
| Weeks 2 to 6 | Tenderness gone for most; any tingling slowly improving | Report anything that is not improving; keep other treatments on the area one to two weeks apart |
| Months 2 to 3 | New collagen forming; gradual firming | Review appointment with comparison photographs |
What to avoid
What to avoid after HIFU, and for how long
When injections and HIFU are planned for the same visit, an expert consensus recommends the energy treatment first and the injections afterwards. If your doctor gives different timings for your face, follow them.
Most people can go back to work the same day; the restrictions are about heat, irritation and timing rather than rest.
| Avoid | For how long | Why |
|---|---|---|
| Saunas, steam rooms, hot yoga and very hot exercise | Until any swelling has settled, usually a few days | Heat can keep swelling going |
| Retinoids, acids, scrubs and peels on the area | A day or two; longer if the skin is still red or sore | Treated skin is more easily irritated |
| Unprotected sun | Always, and especially while the skin is pink | Lowers the risk of darkening |
| Fillers, wrinkle-relaxing injections, lasers or another energy treatment on the area | One to two weeks, unless your doctor plans them for the same visit | Lets side effects settle and results be judged |
| Pressing, rubbing or vigorous massage on tender areas | While tender | It adds discomfort; if swelling persists, the clinic can advise |
| Another HIFU session on the same area | Until the review at about three months | The result is still developing, and over-treatment can affect the fat layer |
Special cases
Special cases: fillers, threads, injections, pregnancy and implants
Some histories change the plan or rule HIFU out. Tell the doctor about all of them, with dates, before treatment.
If you are unsure whether something counts, mention it anyway; the doctor can decide whether it changes the depth, the area or the timing, or whether another treatment would suit you better.
- Fillers: the maker has not evaluated treatment over dermal fillers and advises against treating directly over them. Tell the doctor where and when filler was placed; when both are planned for one visit, the energy treatment usually comes first.
- Threads: tell the doctor about any thread lift and when it was done. Lifting procedures can shift how deeper structures sit, which is one reason a 2026 review urged extra care with nerves after earlier facial procedures.
- Wrinkle-relaxing injections: these can follow the energy treatment on the same visit, or be spaced one to two weeks apart, as your doctor plans.
- Pregnancy and breastfeeding: HIFU is not suitable during pregnancy, and the maker has not evaluated it in pregnant or breastfeeding women.
- Implants: active implants such as pacemakers or defibrillators, and metal implants in the treatment area, such as plates or screws from jaw surgery, are reasons not to treat. Treatment is not recommended directly over mechanical or breast implants, and the device is kept at a distance from cochlear implants, neurostimulators, stents and shunts.
- Other conditions: tell the doctor about keloid scarring, blood thinners or bleeding problems, cold sores, autoimmune disease, diabetes, epilepsy, Bell's palsy, numbness in the face, or active acne or infection in the area.

How SKINLIFT® and HIFU at IN Eternity Clinic handle aftercare
HIFU at IN Eternity Clinic, 9 Scotts Road, #12-01 Pacific Plaza, beside Orchard MRT, is performed by Dr Sin Yong on the Ultraformer MPT after a consultation. Numbing cream may be applied first. Afterwards the skin may look a little pink and feel tender to touch for a few days, and you can usually go back to work the same day. The clinic's advice is to use sunscreen, keep skincare gentle for a day or two, and avoid saunas and very hot exercise until any swelling settles. Photographs are taken so that changes can be compared fairly later. WhatsApp +65 8815 3008.
SKINLIFT® at SkinRev Lab, #B1-28 CityLink Mall, is a non-medical, 30-minute session done without numbing cream or injected anaesthetic. Most clients go back to work straight after; mild, short-lived redness or slight swelling can occur, and the Aesthetic Specialist will stop or adjust the settings if you are uncomfortable. The centre advises seeing a doctor first if your skin is very loose, if fullness under the chin is large, firm, one-sided or appeared quickly, or if you have had injections or surgery in the area. WhatsApp +65 8266 9585.
VF Lift, which Dr Sin Yong performs at SkinRev Lab, can include focused ultrasound. Redness and warmth usually fade within hours, the jawline or under-chin area can feel swollen or tender for one to three days, and the advice is gentle skincare, sunscreen and no saunas or hot workouts until the skin has settled.
Lower the risk
How to lower your risk before HIFU
Most serious problems after HIFU trace back to the wrong candidate, the wrong depth or rushed technique. A few steps before you book lower the odds.
None of this removes risk entirely. A clinic that explains what HIFU cannot do, as well as what it can, is usually a good sign.
- Have a doctor assess where the laxity sits, how thick the skin is and how much fat lies over the bone before the first treatment.
- Ask for the device by name, and whether the cartridge or transducer is genuine and in date.
- Mention fillers, threads, surgery, implants, pregnancy, blood thinners, keloids and any facial numbness.
- Postpone if the skin in the area is broken, infected or sunburnt, or has inflamed acne.
- Tell the operator about sharp or burning pain during treatment. Your feedback helps avoid too much heat in one spot, which is one reason some experts are wary of nerve blocks that remove it.
- Book the review at about three months before planning anything more.

Common questions
Your questions,
answered
What are the side effects of HIFU?
The usual ones are redness, mild swelling and tenderness, settling within hours to days, with tenderness to touch lasting up to two weeks. Less common are bruising, raised lines, tingling or numbness. Rare problems include temporary facial muscle weakness, burns, darkening and fat loss in a thin face.
How long does swelling last after HIFU?
Usually from a few hours to three days, and up to about a week in some people. Swelling that increases after the third day, or comes with heat, pain or fever, should be checked by the clinic.
Is numbness or tingling normal after HIFU?
It can happen when heat irritates a small sensory nerve, and the maker reports it typically resolves within two to six weeks. Patchy numbness that steadily improves is expected; numbness that is not improving, or any muscle weakness, should be reported.
Can HIFU damage facial nerves?
Temporary nerve effects are rare. Weakness of a muscle, such as an uneven smile, typically recovers within two to six weeks, and no permanent facial nerve injury occurred in the maker's trials. Reports after launch describe effects lasting months, so report any weakness the same day.
Can HIFU cause fat loss?
It can in some cases, especially in a thin face that is over-treated, although published cases are few. Reports to the US FDA list fat loss among the most frequent problems for one device, so depth and density should be planned around how much fat you have.
What should I avoid after HIFU?
Avoid saunas, hot yoga and very hot exercise until swelling settles, strong actives such as retinoids and acids for a day or two, and unprotected sun. Space fillers, injections or other energy treatments one to two weeks apart unless your doctor plans them together.
When can I exercise after HIFU?
Light activity is usually fine the same day. Very hot exercise, saunas and hot yoga are better left until any swelling has settled, which usually takes a few days.
Can I have HIFU after fillers or wrinkle-relaxing injections?
Often, yes, but tell the doctor where and when. The maker advises against treating directly over fillers, and an expert consensus puts the energy treatment before injections on the same visit, or spaces treatments one to two weeks apart.
Is HIFU safe during pregnancy?
HIFU is not suitable during pregnancy, and it has not been evaluated in pregnant or breastfeeding women. Wait until after pregnancy and breastfeeding, and keep to sunscreen and gentle skincare in the meantime.
